STATEMENT OF COOPERATION

 

I/We, the parent(s) of the aforementioned student, am (are) applying his/her enrollment for the following school year.  The completion of this application and inclusion of all applicable fees is the “first step” in the re-enrollment process. NOTE: Maintenance Fee, Tuition Fee and Registration Fee and other fees established are non-refundable (will not be returned for any reason).  With the submission of this application, I/we also acknowledge our agreement and cooperation with the Philosophy, Purpose, and Principles (as set forth in the Student Handbook) of Carib Christian School.,

 

I/We agree with the tuition and all fee payment arrangements as established by this institution.

 

I (We) understand the importance of our financial obligation, and agree with certain stipulations as follows: If my account balances have passed the 15th day beyond the “due date”, it is fully understood that my child(ren) shall be subject to suspension, removed from the right to take tests and logged off the school’s online grading system, with the exception of December and May, in which case, due to semester closings, the last day of payment will be considered the 10th day of both months without any extended period.  The account must be brought completely up to date before re-admission; which means no pending balance at all. (NO EXCEPTIONS)

 

I (We) renounce all rights to claim transcripts or grades and accept that ALL MONIES “owed” by student or parent shall be “just reason” to withhold all transcripts and/or grades by administration (Administrator shall have final word in this matter).

 

I (We) as parent(s) of the aforementioned child shall also accept the time schedule of CCS.  No child shall be “dropped off” before 7:30 AM or left after 3:30 PM.  With the exclusion of parents under the “After School Care Agreement”, any student remaining after 3:30 will automatically be charged a fee of $5.00 per half hour; to cover for after school care. (NO EXCEPTIONS)

 

I (We), as legal guardian(s), assume upon ourselves and release CCS from all civil liability and responsibility related to their care and protection if I (we) leave him/her/them on the CARIB CHRISTIAN SCHOOL premises prior to 7:30 AM or after 3:30 PM without having signed a contract under the “After School Care Agreement”.

 

I/We, the parent(s) of the aforementioned child, also understand that it is the parent’s/student’s responsibility to find out what, if any, class work/homework he/she has missed when absent from class (NOT TEACHER).

 

I/We realize that as a Christian not for profit institution and as such, we are expected to assist with all school fundraisers and extracurricular activities.

 

I/We hereby agree not to divulge the information in this contract to a third party.

1. I (we) am (are) applying for the enrollment of said student for the 2024/2025 school year.  The completion of this application, and inclusion of all applicable fees, is the “first step” in the enrollment process.  NOTE: Registration fees and/or deposits are nonrefundable (will not be returned for any reason).  NO EXCEPTIONS.  It is to be understood that Registration fees include: Maintenance Fee, Entrance Fee, Registration Fee, Tuition Fee, Summer Fee, and other fees established.

1. Yo (Nosotros) estoy (estamos) solicitando ingreso para el estudiante arriba mencionado para el año escolar 2024-2025. Completar esta solicitud y la inclusión de todos los gastos son los primeros pasos en el proceso de matrícula. NOTA: El pago por concepto de Cuotas de matrícula y/o depósitos no se reembolsará por ninguna razón (entiéndase por cuotas: Cuota de Mantenimiento, Matrícula, Cuota de Admisión, Cuotas de Verano, etc.).

2. With the submission of this application, I/we also acknowledge our agreement and cooperation with the Philosophy, Purpose, and Principles as well as the rules and regulations as set forth in the Parent/Student Handbook of CARIB CHRISTIAN SCHOOL, ministry of “Iglesia Rey de Reyes”, which I/we certify to have received with this package.

 2. Al someter esta solicitud y el pago, yo (nosotros) entiendo (entendemos) que estoy de acuerdo (estamos de acuerdo) y cooperaremos con la filosofía, el propósito, y los principios, al igual que con las reglas y reglamentos establecidos en el Manual del Padre/Estudiante de CARIB CHRISTIAN SCHOOL, ministerio de Iglesia Rey de Reyes, el cual certifico (certificamos) que he (hemos) recibido.

 

3.  I (We) agree with the tuition and fee payment arrangements as established by this institution, therefore, I (we) recognize that this is an annual contract for which I (we) understand that we are obligated to pay for the entire school year, having CARIB CHRISTIAN SCHOOL  the right to require payment in full and understanding that CARIB CHRISTIAN SCHOOL is not obligated to refund monthly installments paid, even if the student is removed from school for any reason. In the eventuality that I (we) do not pay the monthly payment which is, in fact, a pro-rated portion of the yearly fee contracted for tuition on or before the 1st day of each month and late after the 10th day of each month. I (we) agree to pay a late fee of $35.00 per child whose tuition is not paid on or before the due date.  It is the parent’s responsibility to pay this fee whether the bank collects it or not. Payments will be considered late if they are made in the bank on the afternoon of the 10th day of each month after bank close their deposit dates. I (We) recognize that CCS will not be responsible for bank policies in regard to time of closing accounts and other transactions to be registered as next day payments.  

3. Estoy (Estamos) de acuerdo con la mensualidad y los pagos establecidos por esta institución.  En el momento en que yo (nosotros) no pague (paguemos) la mensualidad (la cual es una porción del pago contractual anual por concepto de matrícula) en o antes del día diez de cada mes, yo (nosotros) acepto (aceptamos), pagar la cantidad de $35.00 por niño por concepto de demora.  Es responsabilidad de los padres pagar esta cuota por concepto de demora, aunque el banco por error u otra razón no la cobro. Se considera un pago tardío aquel realizado en el banco, durante horas de la tarde del mismo día 10 de cada mes, si la hora de depósito es posterior a la hora de cierre de caja del banco. Yo (Nosotros) reconozco (reconocemos) que CCS no es responsable por las políticas del banco sobre el tiempo y hora de cierre de cajas y transacciones a ser registradas como gestiones hechas en día posterior.

4. I (We) understand/agree on the following:

a. If I (we) take advantage of the Thanksgiving Special Registration Discount offer, I (We) agree to pay a deposit of $250 for each child within 48 hours of submitting this document which will be applied to the Entrance / Maintenance Fee. The deposit must be paid on or before November 22, 2023. This deposit is non-refundable. I (We) understand that I (we) will receive a discount towards the corresponding fees, if the remaining balance of the enrollment fee is paid in full on or before March 20, 2024 and/or while the offer lasts.

b. If I (we) take advantage of the Christmas Special Registration Discount offer, I (We) agree to pay a deposit of $250 for each child within 48 hours of submitting this document which will be applied to the Entrance / Maintenance Fee. The deposit must be paid on or before December 15, 2023. This deposit is non-refundable. I (We) understand that I (we) will receive a discount towards the corresponding fees, if the remaining balance of the enrollment fee is paid in full on or before April 5, 2024 and/or while the offer lasts.

c. If I (we) wish to save my child's space for the next school year 2024-2025, I (We) agree to pay a deposit of $250 for each child within 48 hours of submitting this document which will be applied to the Entrance / Maintenance Fee. The deposit must be paid on or before January 31, 2024. This deposit is non-refundable. I (We) understand that if I (we) do not pay the deposit and complete the registration process on or before January 31, 2024, Carib Christian School cannot ensure your child will have a space for the next school year 2024-2025. I (We) agree to pay a late fee of $100.00 per family if the remaining balance is not paid on or before May 1, 2024.

 

4. Entiendo (Entendemos) y estoy (estamos) de acuerdo con lo siguiente:

a. Si yo (nosotros) aprovecho (aprovechamos) la oferta de Acción de Gracias de Descuento Especial de Matrícula, acepto (aceptamos) pagar un depósito de $ 250 por cada niño/a dentro de las 48 horas posteriores de haber sometido este documento que se aplicará a la Cuota de Entrada / Mantenimiento. El depósito debe pagarse antes del 22 de noviembre de 2023. Este depósito no es reembolsable. Entiendo (Entendemos) que recibiré (recibiremos) un descuento en las tarifas correspondientes, si el saldo restante de la cuota de matrícula se paga en su totalidad en o antes del 20 de marzo de 2024 y/o mientras dure la oferta.

b. Si yo (nosotros) aprovecho (aprovechamos) la oferta de Navidad de Descuento Especial de Matrícula, acepto (aceptamos) pagar un depósito de $ 250 por cada niño dentro de las 48 horas posteriores de haber sometido este documento que se aplicará a la Cuota de Entrada / Mantenimiento. El depósito debe pagarse antes del 15 de diciembre de 2023. Este depósito no es reembolsable. Entiendo (Entendemos) que recibiré (recibiremos) un descuento en las tarifas correspondientes, si el saldo restante de la cuota de matrícula se paga en su totalidad el 5 de abril de 2024 o antes y/o mientras dure la oferta.

c. Si yo (nosotros) deseo (deseamos) guardar el espacio de mi hijo/a para el próximo año escolar 2024-2025, acepto (aceptamos) pagar un depósito de $ 250 por cada niño/a dentro de las 48 horas posteriores de haber sometido este documento que se aplicará a la Cuota de Entrada / Mantenimiento. El depósito debe pagarse antes del 31 de enero de 2024. Este depósito no es reembolsable. Entiendo (entendemos) que si no pago (pagamos) el depósito y no completo (completamos) el proceso de inscripción en o antes del 31 de enero de 2024, Carib Christian School no puede garantizar que su hijo/a tenga un espacio para el próximo año escolar 2024-2025. Yo (Nosotros) acepto (aceptamos) pagar un cargo por pago atrasado de $100.00 por familia si el saldo restante no se paga en o antes del 1 de mayo de 2024.

 

5. Should the bank in which I (we) have our checking account return a check, I (we) agree to pay the amount of $25.00 to cover the administrative cost of record keeping and any penalty imposed by the bank institution, in addition to the amount of the check. I (We) declare knowledge of the law and certified I (We) know it’s a crime submit checks without funds. I (We) recognize too, that according to Puerto Rico legal system, with the exception of certified checks, a check is considered a payment offer, so I (We) authorize CCS to maintain an unpaid balance until the check is cleared and cashed out.  

 5. Si el banco en el cual tengo (tenemos) la cuenta de cheques le devuelve un cheque a la escuela, yo (nosotros) estoy (estamos) de acuerdo en pagar la cantidad de $25.00 para cubrir los gastos administrativos ocasionados a la contabilidad, además de cualquier penalidad que imponga la institución bancaria, y en adición a la cantidad del cheque. Yo (Nosotros) declaro (declaramos) conocimiento de la Ley y certifico (certificamos) que conozco (conocemos) que hacer entrega de pagos mediante un cheque sin fondos bancarios es un delito. Yo (Nosotros) reconozco (reconocemos), que según el sistema legal de Puerto Rico reconoce, con la excepción de cheques de cajero o certificados, un cheque es considerado una oferta de pago, por lo que yo (nosotros) autorizo (autorizamos) a CCS a mantener un registro de pago no realizado hasta que el banco certifique que el dinero fue liberado y el efectivo recibido.

 

6. Yo (nosotros) entiendo (entendemos) la importancia de nuestra obligación financiera y acepto ciertas normas/reglamentos como: Si el balance de mi cuenta pasa 15 días de su fecha límite, se entiende que el niño estará expuesto a ser suspendido, removido su derecho a tomar exámenes, y no tendrá acceso al sistema de calificaciones en línea de la escuela, con excepción de diciembre y mayo, fechas en las cuales, debido al cierre del semestre, el último día de pago será considerado el día 10 de ese mes, sin extensión alguna de tiempo.  La cuenta deberá ponerse al día antes de ser readmitido; lo que significa ningún balance pendiente (SIN EXCEPCIÓN).

 

7. Should I (we) withdraw my (our) child (children) from CARIB CHRISTIAN SCHOOL, I (we) agree to clear all financial responsibilities before the release of any transcripts, recommendation letters, etc.  The first transcription will be released without any cost, if all financial responsibilities were cleared. A $10.00 fee will be charged for any additional transcript and a $15.00 fee will be charged for recommendation letters.

7. Si yo (nosotros) retiro (retiramos) mi(s) hijo(s) de CARIB CHRISTIAN SCHOOL, yo (nosotros) acuerdo (acordamos) pagar toda responsabilidad financiera previo a que la escuela me entregue cualquier reporte o transcripción de notas, carta de recomendación u otro documento oficial. Se hará entrega de una primera transcripción libre de costos., si todas las obligaciones económicas han sido saldadas. Un cargo de $10.00 será requerido por cada transcripción adicional y un cargo de $15.00 será requerido por cada carta de recomendación.

 

8. In the case of the annual tuition fee and the after-school care, the fee may be adjusted according to the operational costs or requirements of the regulatory and/or governmental agencies.

8. En el caso de la anualidad y el horario extendido, los costos podrán ser ajustados de acuerdo a los costos operacionales o exigencias de las agencias reguladoras y/o gubernamentales. 

 

9. Upon the withdrawal of my child, I (we) am (are) aware that if I (we) decide to enroll him/her back to CARIB CHRISTIAN SCHOOL after the end of two (2) school years, my (our) child will be considered as a new student, and therefore, will be required to pay the Entrance Fee in full.

9. Luego de haber dado de baja a mi hijo/hija, estoy consciente de que, si decido matricularlo luego de haber pasado dos años escolares, se considera como un estudiante nuevo y tendré que pagar el costo de entrada en su totalidad.

 

10. I (We) renounce all rights to claim transcripts or grades and accept that ALL MONIES “owed” by student or parent shall be “just reason” to withhold all transcripts and/or grades by administration (Administrator shall have final word in this matter).

10. Yo (Nosotros) renuncio (renunciamos) a todo derecho a reclamar transcripciones o notas y aceptamos que todo dinero que el estudiante deba se considera razón suficiente para retener las transcripciones y/o notas. (La Administración tiene la última palabra sobre este particular).

 

11. Yo (Nosotros) como padre(s) del mencionado estudiante, acepto (aceptamos) el horario de CARIB CHRISTIAN SCHOOL.  Ningún niño se dejará en la escuela antes de las 7:30 AM ni se recogerá después de las 3:30 PM. Con la excepción de los padres bajo el acuerdo de cuidado luego de clases (“After School Care Agreement”), todo estudiante que permanezca en la escuela después de la 3:30 PM se le cobrara $5.00 por cada media hora; para cubrir los gastos de cuidado después de clases (SIN EXCEPCIONES). 

 

12. I (We), as legal guardians, assume upon ourselves and release CCS of all civil liability and responsibility related to their care and protection if I (we) leave him/her/them on the CARIB CHRISTIAN SCHOOL premises prior to 7:30 AM or after 3:30 PM without having signed a contract under the “After School Care Agreement”.

Yo (Nosotros), como guardián o tutor legal del o los menores, asumo toda responsabilidad y libero a CCS de cualquier demanda o responsabilidad civil relacionada con el cuidado y protección de mi niño (niños), si lo (s) dejo (dejamos) en los predios de CARIB CHRISTIAN SCHOOL antes de las 7:30 AM o después de las 3:30 PM. (SIN EXCEPCIONES).

 

13. I (We), the parent(s) of the aforementioned child, also understand that it is the parent’s/student’s responsibility to find out what, if any, class work/homework he/she has missed when absent from class (NOT THE TEACHER). I (We), recognized that all teachers have their professional period and we should adjust to that time to have discussions about any matter, and they are not obligated to attend parents out of this time or replace lost grades for absence, with the exception of medical reasons with a doctor certificate.

 13. Yo (Nosotros) el padre (los padres) del mencionado niño, también entiendo (entendemos) que es responsabilidad del (los) padre(s)/estudiante verificar los trabajos/tareas que este ha perdido por haber estado ausente de clases. (No es responsabilidad del maestro). Yo (Nosotros), reconocemos que todo maestro tiene su hora profesional y debemos ajustarnos a su itinerario para la discusión de asuntos de los estudiantes, no estando obligados los maestros a atender padres fuera de esas horas profesionales, ni a la reposición de notas perdidas por ausencia, salvo aquellas razones médicas para las que se presente certificado médico.

 

14. I (We), the parent(s) of the aforementioned child also understand that we are required to cooperate with all school fundraising activities.

14. Yo (Nosotros) el padre (los padres) del mencionado niño, también entiendo (entendemos) que se requiere de nuestra cooperación en todas las actividades (incluyendo ventas de recaudaciones de fondos) relacionadas con la escuela.

 

15.  I (We), the parent(s) or legal guardian(s) of the aforementioned child fully understand that as part of this contract and agreement, I (We) must purchase the official CCS uniform under these clauses and conditions:

 

a. All full uniforms will be bought only at Madison Store in Aguadilla Mall, including polo’s, jackets, pants and PE shirts. All uniform pieces (tops and bottoms) have the Carib logo imprinted; therefore, Carib has the right to refuse service to attending students without the proper uniform (NO EXCEPTION)

b. All special sizes should be requested with prior orders. Carib or Madison does not guarantee these special orders if they are not made at least 180 days before classes start.

c. It is required that students use PE uniforms and sneakers on PE days, and formal uniform the rest of the week.

d. The care and cleaning of the uniform will be the parent’s responsibility under manufacturer instructions. 

 

15. Yo (Nosotros), el padre (los padres) o tutor (es) legal (es) del mencionado niño, entendemos en su totalidad que como parte de este contrato y acuerdos, yo (nosotros) debo (debemos) adquirir el uniforme oficial de Carib bajo las siguientes clausulas y condiciones:

 

a.       Todo uniforme completo tiene que ser adquirido de la tienda Madison localizada en “Aguadilla Mall”, incluyendo polos, pantalónes, abrigos y camisas de la clase de educación física o PE. Toda pieza del uniforme (camisas o pantalones) tiene el logo de Carib impreso, razón por la cual, Carib retiene el derecho a negar servicio a estudiantes que comparezcan sin el uso del uniforme apropiado. (SIN EXCEPCIONES)

b.        Todo tamaño especial deben ser ordenado previamente. Carib y Madison no pueden garantizar órdenes especiales sino son formalizadas al menos 180 días previos al inicio de clases.

c.       Como parte de los requisitos del uniforme, el estudiante debe utilizar uniforme de PE y zapato deportivo los días de clases de PE, pero uniforme formal el resto de la semana.

d.        El cuidado y limpieza del uniforme será responsabilidad de los padres bajo las instrucciones del manufacturero.

 

16.  I (We)understand Carib Christian School is a regular private academic institution that does not provide special education services. 

16.  Yo (Nosotros) entiendo (entendemos) que Carib Christian School es una escuela privada de corriente regular académica que no brinda servicios de educación especial. 

 

17.   I (We) understand that Carib Christian School is an educational institution which teaches in the English language,

so any setback the student has due to language must be attended to by parents privately, even when such attention requires the hiring of tutors or education professionals, which Carib does not commit to provide.

17.  Yo (Nosotros) entiendo (entendemos) que Carib Christian School es una institución de educación completa en el idioma Inglés, por lo que cualquier retraso que el estudiante tenga por razón del idioma, o cualquier otro, deberá ser atendido por los padres privadamente, aún cuando dicha atención requiera la contratación de tutores o profesionales de la educación, lo que Carib no se compromete a proveer.

 

18.  I (We) understand that in the event of a natural phenomenon and/or any other situation which would prohibit Carib Christian School from continuing to provide face-to-face classes, the school will automatically convert to providing an online class structure until it is safe to return to the physical classroom. The cost of the annual tuition fee may increase by 10% during the time that online classes are offered.

18.    Yo (Nosotros) entiendo (entendemos) que en el caso de un fenómeno natural y/o cualquier otra situación que prohíba que Carib Christian School continúe brindando clases presenciales, la escuela se convertirá automáticamente en una estructura de clase en línea hasta que sea seguro regresar al colegio. El costo de la anualidad podrá aumentar un 10% durante el tiempo que se brinde clases en línea.

 

19.    I/We hereby agree not to divulge to a third party the information in this contract or any other school document.

19.   Yo (Nosotros) el padre (los padres) o tutor (es) legal (es) del mencionado niño, acordamos no divulgar información de este contrato, ni cualquier otro documento escolar, con terceras personas.

Carib Christian School gladly announces its website, www.caribchristianschool.com as well as other social media as being a part of our promotional services.  Our parents will now have fingertip access to news, calendar events, school information, as well as, links to great sites for educational resources, games, tech news and much more.

As you know, Carib takes photographs and videos during most of its school activities.  If photographs or videos are taken, this would be for the purpose of educating students, promoting the school, and informing our families of school activities.  Because we respect each family’s right to privacy, we will not post students’ or parent’s pictures on the website or our web pages if you do not agree to it.

Therefore, we request your permission to allow the school to publish photographs or videos of your child and/or samples of work done by your child which could include, but are not limited to, www.JupiterEd.com, school website, school newsletters (online and in hard copy), social media, posters and/or promotional materials.  If published as part of the above mentioned circumstances, third parties would be able to view the photographs and work made by our students’.

We hope you consent to this arrangement by selecting “I agree” in the agreement form below.  By accepting you will be in agreement to the following:

1.       The school is able to publish photographs and videos of your child and samples of child’s work as many times as it requires in the ways mentioned above.

2.       Your child’s photograph and video may be reproduced either in color or in black and white.

3.       The school will not use your child’s photograph, video, or samples of your child’s work for any purpose other than for the education of students, to announce school activities or for the general promotion of the school.

4.       This consent will remain effective until such time as you advise the school otherwise.

Any photographs and videos taken by the school will be stored and disposed of securely.

If you agree to permit the school to take photographs and videos of your child, and to publish photographs and videos of your child, or samples of your child’s work in the manner detailed above, please complete and sign the consent form.

***If this form is not submitted with a denying consent by the above mentioned date, Carib will understand that you agreed with this with your child’s formal inscription form.

Given that the Parent/Student Handbook contains essential information about Carib Christian School, it is required that every parent/guardian and student read its contents.  Parents may read to younger students the appropriate sections relevant to their role here.

 

Parents/guardians and students must sign below to indicate that they have read the Handbook, discussed it carefully, and agree to comply and be governed by the philosophy and rules of Carib Christian School.  Signatures indicate your understanding that policies and procedures may not be limited to those specifically contained in this Handbook.

 

*This new and revised edition of our "Parent/Student Handbook" is our most recent edition and reflects several very important changes for the administration of the school.  Remember that all parents are to accept this parent/student handbook and its revisions as part of the Enrollment Application Contract.

 

We have read the Parent/Student Handbook and agree to support the school’s mission and Christian philosophy.  We agree to comply with all school policies and procedures.

The purpose of this survey is to collect the socio-economic information of the students. This information identifies the students that can make it possible for the school to qualify for Federal Programs funds. The Department of Education and the Federal Affairs Office assigns funds to provide educational services to students in kindergarten through twelfth grade. To complete the federal proposal documents, we are required to submit a summary of the socio-economic study. Please complete one sheet for each student and return it. The information we request is confidential. You do not need to write your name.

El propósito de esta encuesta es recoger la información socioeconómica de los estudiantes. Esta información permite identificar los estudiantes que hacen posible que el colegio cualifique para los fondos de Programa Federales.
