USTA MAKE-UP MATCH PUBLIC COURT REQUEST

This form is for use by USTA teams playing home matches on the City of Durham public courts exclusively. DPR Field Rentals receives and processes requests for courts Monday-Friday. Please allow a minimum of 3 business days for a response. Ensure the information submitted in this form is accurate and that all players have agreed to the dates and times requested. Please email coordinator@enoctanc.com with questions.
The Original Courts were canceled within 24 hours of the match date. If courts were not canceled within 24 hours you cannot request make-up courts.*
