  • Congratulations on your upcoming wedding, and thank you for choosing Jo Sharkey Makeup to help celebrate your special day! 

    You are ready to set up your makeup appointments and I am ready to assist you. The purpose of this agreement is to confirm the services to be rendered for the wedding party along with the name(s) of those responsible for services provided by Joanna Sharkey. This agreement must be completed and returned before the services will be scheduled and the time will be reserved. All bookings are on a “first-come-first-served” basis. No dates will be guaranteed without a signed contract and a deposit to secure them. The deposit is £60 and is none refundable as per the terms and conditions, this will be deducted from your final bill.

  • • Please arrive for your services on time if you are selecting in salon services

    • Please consider bringing your own skincare if you have allergies

    (eg. moisturiser and a cleanser)

    • Wearing a button-down shirt or a robe is suggested

     

  • Bridal Part Information

  • Please ONLY include information about wedding guests that will be receiving services with me. Any information with a star next to it needs to be filled out if this does not apply please put N/A so you can move to the next question.

    If you can't insert a picture of the people wanting makeup in this form please email or message pictures of your bridal party to me this helps me determine what I will need on the day of the wedding. 

    You must tell me if any of the bridal party has allergies including but not limited to latex and ingredients for example nut allergy, any product allergic to my. It is your responsibility to ensure that your bridal party gives this information. 

  • Consent for minors

    Consent from the legal parent or guardian of the child/children under the age of 16 years old must be given in order for makeup services to be completed. If you are not the legal parent or guardian you must have consent from the parent or legal guardian of the child or children. The form will be sent to you in a separate link this must be signed and submitted along with this agreement.
  • Signature of parent guardian

    Please note a separate form will need to be signed by the legal guardian of the child or children. Your signature below is your confirmation for me to do makeup on the minor..

  • Makeup preview Schedule

  • I highly recommend a wedding makeup preview to be scheduled at least 4 weeks prior to your wedding day. 

    The preview can be scheduled by emailing Joanna.sharkey84@gnail.com

    During your preview I recomend that you have:

    • Pictures of makeup looks that you like

    • A picture of your dress

    • Also if you can wear a white top
    • Wear a button down shirt or robe 
    • Have information on tanning if applicable like the shade of tan as this will directly affect the makeup look, any semi or permanent makeup you will have on the day including semi permanent lash extensions

    Information in order to guarantee your appointment times, I require the following credit card information. Please understand that you are guaranteeing all of your parties’ appointments, not just your own.
  • A deposit payment of £60 minimum for parties of 5 people, for more than 5 the charge is 25% of your full amount, is required at this stage and to be paid via bank transfer to: Please note if your wedding party is more than 5 people this deposit will be higher dependant on the number of people required for makeup, this. Is charged at 25% of the overall cost and is a non-refundable deposit.

    Once this form and the deposit is made your wedding date will be booked and an invoice sent to you via the email address provided in this form. 

    The remaining balance will need to be paid no later than two weeks (14 days) advance of the wedding date. In the event that an appointment is canceled less than one week (7 days) prior to your appointment, you will be held responsible for ALL (100%) of the cost of the service(s) and billed for the amount. 

    In the event that an appointment is canceled from two weeks (14 days) prior to the date of the wedding of the cost of the service(s) will be charged and invoiced at (50%) of the overall service cost.

    Full payment for all services is due 14 days before your wedding date.

  • Cancellation Policy

  • A cancellation of the service must be made 30 days in advance of the wedding date and notifyed by 30 days before the wedding date. If a cancellation is made after this time you will be charged at 100% of the service cost.

    In the event that you have notified me in 45 days prior to the date of the wedding a 50% charge of the overall service fee will be debited to you.

    If you have notified me of a cancellation prior to this you will not be charged and any payment will be refunded back to you. 

    The deposit payment of £60 is none refundable and covers the booking, if you have paid a deposit of more than £60 due to the size of your group then that deposit will be non refundable and not part of the cancellation refund. 

     

  • Minimum requirements

    There is a minimum of 3 full paying adults to book a wedding party. If you have a large party of 8 or more people I will need an assistant makeup artist to help which I will provide as part of my service to you.
  • Other requirements

    Should any of your bridal party decide they no longer want makeup on the day after this contract is signed, you will need to replace the person/s to makeup the numbers agreed. Your price will remain the same unless the wedding has been cancelled. If the wedding has been cancelled the cancellation policy will apply in cancellations, will apply.
