Reason for excursion/ educational outcomes: Who does'nt love to go bowling? Our children love going somewhere different and trying new things and they do love a healthy competition between friends. Our children feel safe and secure being able to be with their friends bowling and engaging in some healthy competition in the arcade area.

Address of destination: Big Top shopping Centre, Maroochydore 4558, QLD

Proposed activities to be undertaken: A game of bowling and 1 hr arcade time followed by lunch with friends.

·I give permission for my child to attend the above excursion provided they meet the behaviour expectations of our service and this decision will at the discretion of the Co Ordinator.

·My child is healthy and physically able participate in the excursion outlined above.

·I agree that my child will be on a bus with a fitted seatbelt that he/she must wear.

·I understand any medications that are required to be administered on the day of the excursion will require a medication form to be completed and given along with the medication to the ‘responsible person in charge’.

·I understand that a risk assessment has been prepared and is available at the centre as per your request.
