In order to help me streamline the booking process, please fill out any relevant information I may need for your appointment below. If there is a question you do not understand or are not sure how to answer, give a best guess and let me know at the end how I can help. Once this has been filled out, you will be automatically put onto a waitlist and from then, you will be contacted for more information. This can take some time. Preferably picture of you, made in a good light (use timer on your phone). It has to be clearly understamdable, what part of the body it is. Please avoid weird angles and distortion. Mark approximate size and area of the tattoo, either by marker on skin or in photo editor.

If there is a cover-up involved, please take a well-lit photo of the tattoo or scar that you would like to be covered as well as some area around it. Please note how old the tattoo/scar is in the description box.

STYLE*
Use following adjectives: bold, heavy, dark, light, feminine, realistic, illustrational, abstract, dotwork, mix, your choice.
B/G or COLOR*
Color, black and grey, mix.
DESCRIPTION*
If you would like script in your tattoo, please find a font on www.dafont.com, list it below, and what the exact wording will be.
Please make sure the spelling and script is correct.
REFERENCES

Make sure you explain what do you like and what you don't like in the following references. Example: I like this species, but not the pose it is on reference.

Also include one picture of my work, where you like the execution style.

If it is a design from my flash - add a picture where you mark the design, or cut it out to avoid misunderstandings.
