By signing below, I, the undersigned client, acknowledge and agree to the following:
- Makeup Services Only
I understand that services provided are limited to cosmetic makeup application only (e.g., foundation, eyes, lips, contouring, lashes).
No skincare treatments (such as facials, waxing, threading, chemical peels, or any esthetician or cosmetology services) will be performed.
- Freelance Artist
I understand that Victoria Getman operates as a freelance makeup artist in Ohio.
- Legal Compliance
I acknowledge that, under Ohio law, makeup artists providing cosmetic makeup only do not require a license.

- Hygiene & Safety
I understand that all makeup application will use single-use or sanitized tools to ensure hygiene and safety during the service.
- Client Responsibility
I agree to inform the artist of any allergies, skin sensitivities, or medical conditions that could affect the makeup application process.
I release the artist from any liability related to allergic reactions or skin irritations that may occur.
- Cancellations & Policies
I understand that early (before 8:00 AM) and late (after 6:00 PM) appointments require prepayment.
I agree to follow the cancellation and rescheduling policies set by the artist.
I have read and fully understand the terms above. I consent to receiving makeup application services in accordance with Ohio freelance makeup artist regulations.
