1. Services. The Coach agrees to provide health coaching services to the Client as described in Exhibit A attached hereto (the "Services" The Services may include, but are not limited to, providing information on food, lifestyle, supplements and other approaches to help the Client better integrate such into their health program. 2. Disclaimer. The Coach is not a licensed medical professional and is not qualified to diagnose or treat any medical conditions. The information provided by the Coach is for educational purposes only and is not intended to be a substitute for professional medical advice, diagnosis, or treatment. The Client should always seek the advice of a physician or other qualified health provider with any questions regarding a medical condition. Supplement statements have not been evaluated by the Food and Drug Administration. Supplements are not intended to diagnose, treat, prevent or cure any disease. 3. Payment. The Client agrees to pay the Coach the fees set forth in Exhibit B attached hereto for the Services. 4. Term and Termination. This Agreement shall commence on the Effective Date and shall continue until terminated by either party upon [one] days' written notice.
5.Confidentiality. The Coach agrees to keep all information provided by the Client confidential and to use such information only for the purpose of providing the Services. 6. Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the State of Oklahoma, U.S.A. IN WITNESS WHEREOF, the parties have executed this Agreement as of the Effective Date.
