• Roams to Homes Adoption Application

  • Roams to Homes (R2H) is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of stray, neglected and abused animals. R2H is completely funded through fundraising efforts and public donations. All of our pets are cared for by foster homes and receive medical care and are up-to-date on vaccines.  R2H makes no guarantees or statements regarding the dog's age, breed, health, or temperament. While R2H made every effort to provide accurate history and assessment of the dog, we are not able to guarantee the dog’s age, breed, medical status, behavior or disposition.

  • Do you agree to have dog seen by a licensed veterinarian within 14 days of bringing the dog home? I understand that I assume full responsibility for the welfare of this pet from the date of adoption.*
  • If this is a long distance adoption, and you are unable to keep your dog, are you willing to assume the responsibility, including travel, boarding and financial responsibility, for returning the dog to R2H?*
  • If your dog is not spayed/neutered at time of adoption, do you agree to spay/neuter your dog within six months of the adoption date?*
