Disclosure Agreement
We would like to express our gratitude for entrusting us with the important task of assisting you with your income tax matters. In order to ensure a clear and comprehensive understanding of our services and protocols, we are providing you with essential information regarding the services we will perform on your behalf. By signing this document, you acknowledge the accuracy and truthfulness of all the information in your 2023 federal and state tax return. This includes, but is not limited to, details about your earned and unearned income, expenses, deductions, dependents, and other relevant information.
This letter signifies our mutual agreement, and we kindly request that you review its contents thoroughly before signing to confirm the accuracy of all the information contained herein. It is your responsibility to supply us with all the necessary information to prepare your tax returns accurately and completely. We will not independently verify the data you provide, although we may seek clarification on certain details. By signing this letter, you affirm that any verbal information provided to us for tax purposes is true and correct to the best of your knowledge.
For clients claiming the Earned Income Tax Credit, Additional Child Tax Credit, Education Credit, or Head of Household filing status, you confirm the accuracy of all questions answered and information provided related to these credits, as well as information about dependents that determine your eligibility for these credits.
Please be aware that this information will be shared with the IRS and relevant state departments of revenue. Providing false or misleading information for tax filing purposes is unlawful and subject to penalties.
Tax returns may be subject to examination by various taxing authorities at both the state and federal levels. In the event of an examination or audit, you may be asked to provide supporting documents or records to substantiate income or deductions. If an examination or audit occurs, EZ Tax Co is ready to assist you throughout the process, but it is crucial to notify us promptly and forward all relevant communications to us. If an amendment is required due to an error or omission on your part, fees may apply.
Before your return is submitted, you will have the opportunity to review all provided information and confirm its accuracy. After signing your tax return, it will be either electronically filed or mailed to the IRS and state agency without delay. Any corrections or additions after submission will necessitate an amendment. If an amendment is needed due to our error or omission, there will be no additional cost to you. However, if the amendment is required due to an error or omission on your part, fees may apply. After your returns are submitted and tax preparation fees are paid or agreed to be deducted from your refund, we will provide you with a copy of your return. It is important to carefully review the completed returns, and if any changes are required, please bring them to our attention immediately. We will explain or correct the matter without delay and provide you with new, corrected copies.
Our service fees are based on standard rates for tax matters and the complexity of the return, including the number of schedules and forms required by taxing authorities to complete an accurate return. Invoices are due and payable upon presentation, except in the case of selecting a bank product where fees are deducted from the refund. If the refund is offset for any reason, the balance becomes due, and you will be billed for payment, which will be due immediately.
As your Tax Preparer, we collect and safeguard information provided by you through your tax organizer, worksheets, documents, discussions, and any information developed during the engagement. We are committed to maintaining the confidentiality of this information and will not disclose it without your approval or unless required by law, even if you are no longer a client. We have physical, electronic, and procedural safeguards in place to protect your confidential information.
After delivering your tax return, a fee will be applicable should you request a copy again, including prior years' returns prepared by us. We strongly recommend that you save and store your return on your own devices for future reference.
If you choose to have fees deducted from your refund, your refund will be routed through our tax bank before being dispersed to you via direct deposit, office-printed check, or office-issued reloadable debit card. Your tax return may display our tax bank, Pathward's account, and routing number instead of your own on your 1040 signature page. The details of your banking information and disbursement method will be outlined in the "banking agreement" section of your return.
By signing this letter, you grant us permission to contact you regarding any products we offer via phone, email, fax, and/or letter format. If you prefer not to be contacted, please call us at (877) 398-2926.
If you find the terms outlined in this letter acceptable, please initial below. We genuinely appreciate the opportunity to serve you and eagerly look forward to working on your behalf. Should you have any questions or need further information, please do not hesitate to contact us.
