  • As a volunteer of Gems Uncovered, I agree to abide by the policies and procedures of this organization. I agree that the work I do is on a volunteer basis and that there is no monetary payment.

  • Street Outreach Activity

    You agree that by participating in the Gems Uncovered, Inc. Street Outreach, you do so entirely at your own risk. You agree that you are voluntarily participating in this ministry and use of the facilities, personal automobiles, and assume all risks of injury, illness, or death. Gems Uncovered, Inc., Board of Directors are also not responsible for any of the above or for any loss of your personal property.

     

    Acknowledge

    You acknowledge that you have carefully read this "waiver and release" and fully understand that it is a release of liability. You expressly agree to release Gems Uncovered, Inc., and their Board of Directors from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the trainer or instructor for personal injury or property damage.

    Negligence

    To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence.

     

    Note

    If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.

    By signing this release, I acknowledge that I understand its content and that this release cannot be modified orally.

  • Confidentiality Agreement Form

    It is the policy of Gems Uncovered, Inc. to ensure that the operations, activities, and affairs of the clients and organization are kept confidential, unless the sharing of such information is approved by the President/Founder. Confidential information includes, but is not limited to, the location of any client's placement shared during a volunteer activity, Gems Uncovered, Inc. internal reporting, and Gems Uncovered, Inc. intellectual property. Photos of victims/clients or any photos at events or facilities should not be taken and/or shared on social media without prior approval from the President/Founder.

  • Signature

    By signing this agreement, the individual agrees to the confidentiality of Information Policy and acknowledges that violations of confidentiality will result in disciplinary action and possible termination.
