1st Week Food Handlers Zoom Registration

After you have completed your Food Handlers Course and received your certificate, you are required to get a Food Handlers Card within 120 days of completion of the course. This can be done by either passing a quiz administered in person at our office in Harrisonville or through Zoom. In order to be able to take the quiz over a Zoom call, you must submit this registration form at least 48 hours prior to the test date and be approved to take the quiz online.

Once approved, the link to join the Zoom meeting will be sent to the email you provide below. It will be sent 24 HOURS prior to the quiz start time. To join, click the link found below "Join Zoom Meeting" in the email you receive. You can either launch the meeting through the Zoom application or on a browser. You will be placed in a waiting room once you launch the meeting, and be admitted one at a time to take the quiz.

Note: If you answer "No" to any of the questions below, you will not be able to take this Food Handlers Quiz on Zoom.
