Northern Texas PGA Junior Golfer & Caddie Accommodation Request Form
The NTPGA Junior Tour will, at its sole discretion and upon receipt of applicable medical documentation, make reasonable accommodations for Junior Golfers or Caddies with disabilities or chronic health conditions consistent with the Americans with Disabilities Act (ADA). The NTPGA Junior Tour will evaluate such requests on a case-by-case basis.
