    Caregiver and Emergency Contacts

  • Caregiver 1 Information

    This is the person the camper usually resides with.

  • Caregiver 2 Information

    This is usually the spouse or partner of caregiver 1 OR another person the camper may reside with at times.

  • Emergency Contact Information

    It is important we have a DIFFERENT third person who will be contacted only if we are unsuccessful in our attempts to contact the parents/caregivers listed above.

  •  Child's Swimming Ability

    Some of our camps may include swimming and/or water based activities. It is important our staff know the level of swimming ability to safely plan these activities.

  • Special Diets

    We need to know if your child requires a special or restricted diet whilst at camp.

    • If it is for medical reasons (such as anaphylaxis, allergies, eczema, etc) please advise us.
    • It very important that our kitchen staff know of any severe food allergies and any treatment plans for these food allergies should a reaction occur.
    • Unfortunately we are unable to cater for diets based on "lifestyle choices" at our camps if special meals need to be created for them. 
  • Dietary Requirements Surcharge

    Due to the added cost and increased workload of catering for some dietary requirements (coeliac, GF, DF) we need to charge a surcharge for all campers that require a special menu. This is approximately $25.00

    • This surcharge will be waived for caregivers that are able to contribute food/meals for the camper.
    • We can allocate additional credit off camp fees on a case-by-case basis if caregivers are supplying large quantities for special diets. 
    • We can also supply our menu plan for the week to help caregivers mirror the meals being served, to help campers feel more at home during camp. Please contact the CYC office for more information about options after enrolment (kidscamps@cycwaihola.org.nz)
  • Health Information

  • Please let us know how we can best manage any health issues.

    Here are some examples of health notes:

    Bed Wetting

    • John wets the bed on a occasional basis 
    • Its a good idea to put John on a bottom bunk and a leader chat with him each morning to see if he needs a shower/change 

    ADHD 

    • John has been diagnosed with ADHD 
    • A leader can help explain instructions during activities, John needs to be kep busy so it won't be an issue.
    • Medication should be given at the prescribed times and dosages to keep the ADHD in check.
    • Please try to keep John away from other children with ADHD as he can be wound up by them and will possibly wind them up as well

  • Notification of Medications

    • It is very important that our camp staff know about every medication the camper will be taking during camp, why they are taking it, and when they need to take it during camp.
    • This includes supplements, any prescribed treatment, and antibiotics required over the course of camp.

     

  • Medication List Examples

    Here are some examples of a medication list:

    Nurofen Tablet (80mg):

    - One tablet every 6 hours with food if required due to inflammation
    - John has a knee sprain that may get inflammed during exercise
    - No precautions

    Ritalin (20mg):
    - One 20mg tablet to be taken each day with breakfast & one 20mg tablet to be taken each day with lunch
    - Required for ADHD and general hyperactiveness
    - It is important that the medication must be taken at the correct times and if forgotten, DO NOT take at a different time

  •  Behavioural Information

  • Notification of Behavioural Issues

    • It is very important that our camp staff know of any behavioural issues that could arise during camp.
    • We use this information to manage camper behaviour and suitable leadership. Unfortunately, if we don't know about existing issues and they arise, we may unknowingly send a camper home rather than implement known solutions.

     

  • Examples of a Behavioural Issues List:

    Bullying:

    • John can sometimes bully other children.
    • This relates to him being bullied as a younger child by older siblings. 
    • A stern word given at the start of camp by the leaders or camp director explaining the consequences of bullying at camp, would help John know it is not tolerated.
    • A phone call to parents at the end of day 1 will allow us to ensure there is no problem.

     

    Inattention:

    • John can sometimes not pay attention during group instructions.
    • Having a leader buddied up with John to ensure he understands any instructions given will certainly help.

    Horse Information & Riding Ability

    When a camper is enrolling in a camp with horse activities, it is important that we know the following information.

    Camp Fees and Discounts

  • Family Discount:

    • We offer a 10% discount off camp fees for any campers coming from the same family within a single holiday period. Siblings must live at the same address and attend CYC Waihola Camps within the same holiday period to qualify.
    • Note: both enrolments need to be submitted back-to-back so we have proof of sibling enrolment :)
  • Camp Subsidies

  • There are several camp fee subsidies options available:

    • Work and Income OSCAR subsidies for those that qualify
    • Ministry of Health Carer Support for those that qualify
    • Prison Fellowship helps the children of prison inmates to attend our camps.
    • CYC Waihola Sponsorship fund for all low-medium income families that don't qualify for either the OSCAR or Ministry of Health subsidies
  • Important Information for OSCAR Subsidies

    • Please apply for OSCAR subsidies by calling Work and Income on 0800 559 009 to arrange an OSCAR form to be sent to you and ask for the form request to be put on recordComplete the form (the CYC Waihola office must complete Section C of the form for you).
    • Return it to Work and Income before camp starts.
    • OSCAR Child-care subsidies are applied based on family income and work/study hours and may not cover full camp fees. The balance of any unpaid camp fees will need to be paid by you.

    MSD also has an online tool to help families see what might be available to them in the way of financial assistance (including OSCAR subsidies) please visit https://check.msd.govt.nz/ to see if you might qualify for OSCAR subsidies for our camps. Ensure you select yes for the "Do you or your partner pay, or intend to pay, for this child to go to an OSCAR programme, for at least 3 hours a week?" question for each child you are intending to enrol.

  • Important Information for MoH Carer Support

    • CYC Waihola is a recognised Carer under the Ministry of Health Carer support scheme. You will need to contact the Ministry of Health to arrange for the Carer support forms to be sent to you. CYC Waihola will need to complete a section of this form before carer support is paid.
    • For all other subsidies, please ring the CYC Office (03 417 7120) to talk about whether we support them or not.
  • Important Information for CYC Sponsorships

    • To apply for CYC Sponsorship for camps you must complete either online our Sponsorship Request Form or download a form from here and send it to the CYC Waihola office.
    • All sponsorship must be approved before the start of camps.
    • If you are part of a church, we also ask that you approach your church leadership for camp fee assistance, as our sponsorships are in high demand.
  • Important Information for Prison Fellowship

    • Prison Fellowship helps the children of prison inmates to attend our camps.
    • By selecting this camp fee subsidy option you elect for us to pass on the camper's name, address and caregiver details to Prison Fellowship in order to have the camp fees potentially paid for.
  • CYC Sponsorship Fund

    • CYC Waihola has a small sponsorship fund to help low income families afford to come to camp.
    • This fund is used regularly and often runs low of funds.
    • Donations to this sponsorship fund over $5 over a year are tax deductable

     

  • Notification for Social Workers

  • Payment Terms for Camp Fees

    • In all cases of a successful enrolment into a camp we will email you an invoice for the camp fees. Campers placed on the waiting list will not be issued an invoice.
    • A non-refundable deposit (usually $30 but could be 50% of full camp fees of our more popular camps) is required to secure all enrolments in a camp.
    • Credit card payments can only be made on issued invoices and not at the time of enrolment.
    • A refund of camp fees paid (less the non-refundable deposit) is available if an enrolment is cancelled more than 7 days from the start of the camp date
    • No refund is available if enrolment is cancelled less than 7 days from the start of the camp date.

    We recommend you talk through any of these conditions that apply to the camper with them before they attend our camps to ensure they understand what is required of them.

    All enrolment information supplied is correct to the best of my knowledge

    I acknowledge that this enrolment form information is used for the safe and effective operation of our camps and for possible further contact with the camper after camp.

    I agree to the use of any photos/video containing the camper named on this form for publicity purposes without any remuneration.


    Medical and Health Conditions

    Unfortunately we cannot allow the camper to attend our camps if they have had (a full refund will apply if you let us know):


    • in the 72 hours before you arrive at camp you have had any sickness or symptoms of sickness including: cold / flu-like systems (this includes runny noses, coughs, sore throats or fever), any vomiting or diarrhoea.
      (OR) in the 72 hours before you arrive at camp you have been in immediate contact with anyone with any other sickness or sickness symptoms (OR) if you have you have had a concussion/head injury within 6 weeks of camp starting

     

    I authorise camp staff to:

    • administer all necessary medication as detailed above and minor medical treatment for basic health care (i.e.sticky plasters, etc).
    • seek urgent medical treatment for the camper at my cost should the need arise during camp.

    Both New Zealand Ministry of Health guidelines and CYC Waihola strongly advise you, as a parent/caregiver, to immunise those children in your care before sending them to camp. It is strongly recommended that all campers attending our camps are fully immunised as per the national guidelines for immunisation in NZ. However we understand that choosing not to immunise may sometimes be an informed parental choice for some families and therefore we cannot enforce these recommendations. Therefore, please advise us if your child is not fully immunised for any reason, in case of a medical emergency.

    CYC Waihola regularly has horses on-site and is also in a rural area where tetanus occurs, it is therefore strongly recommended that you check the enrolled camper has an up-to-date tetanus booster immunisation. 
    Behavioural Conditions

    I will not phone the camper named on this form during camp unless in an emergency. If the camper named on this form becomes unwell, disruptive or is not coping with being away from home, we will phone you on the supplied phone numbers.

    I acknowledge that behaviour from the camper that disrupts the program, is harmful to others or destroys property will result in camper being sent home at my expense. Fees will not be refunded and camper may not be able to attend again.
    Camp Program Conditions

    I acknowledge there will be Bible based talks presented throughout the camp at the campers level with the purpose of showing the relevance of the Bible today and to encourage the camper to develop a wholesome Christian life. Prayer will be said throughout the camp and before meals.

    Camp programs often include outdoor activities that may have an element of high risk. CYC Waihola has strict Standard Operating Procedures (available on request) for managing these activities. Activities are run by trained CYC Waihola paid staff or other persons that have undergone comprehensive training in the activity. Activities can be cancelled or postponed at the discretion of CYC Waihola staff at any time for the safety of the camper and CYC Waihola staff.

    These activities may not be suitable for persons with health or behavioural issues that could affect their ability to participate or follow the safety procedures. Any campers going outside the safety procedures and rules can result in serious harm. Activities in our camp programs may include but are not limited to: Air Rifles / Target Shooting, Archery, Archery Tag, Bubble Soccer, Go Carts, Horse Riding, Hunting (teenage camps only), Paint-ball, Tramping and Zip-line.

    I give consent for the camper named on this form to take part in all camp activities, and for transportation of the camper named on this form to and from any off site activities.


    Payment and Refund Terms

    Unless a written arrangement is in place with the CYC Waihola office (or the camper is applying for a subsidy) :
    A $30.00 non-refundable deposit must be paid with all enrolments before the camper is enrolled in the camp
    Camp fees must be paid in full before the camper will be admitted to camp

  • Additional Conditions for Horse Riding Camps

    The Horse Riding activities at CYC Waihola have an element of high risk and as we are committed to keeping our campers safe we advise that all Horse Riding:

    • is subject to availability of trained CYC Waihola paid staff or volunteer ministry staff that have undergone comprehensive training.
    • can be cancelled or postponed at the discretion of CYC Waihola staff at any time for the safety of the campers and/or CYC Waihola staff.
    • campers with health or behavioural issues that could affect their ability to participate or to follow the strict safety procedures given will be prohibited from participating.
    • any campers going outside the strict safety procedures given can result in serious harm.
    • that wearing an appropriate riding hat may reduce the severity of an injury should an accident happen and so agree that I will always wear a riding hat whilst riding or when instructed to do so.
    • I understand and agree that the CYC Waihola will make decisions based on information I provide to them and agree to always be honest and volunteer information about:
      • my abilities and riding experience,
      • any previous riding accidents,
      • any medical condition(s) which may affect my ability to ride.

     

    By submitting this enrolment you agree to the following:

     I understand that horse riding by nature at any level has inherent risk and that any horse may react unpredictably on occasion.

    I understand that I may fall off. I accept that risk and agree that CYC Waihola will not be liable for injury or damage to person or property.
