Quality of Life Assessment Logo
  • Determining if it is time to euthanize your beloved pet can be an incredibly challenging decision. Many times, this decision is made to alleviate unnecessary suffering in well-loved animals. The well-being of pets is determined by their overall physical and mental health, taking into account various aspects of their lives, not just one. The questionaire below aims to encompass all these factors in assessing your pet's life. It's crucial to bear in mind that each pet is unique, and what might be deemed a poor quality of life for one may differ for another.

Input a score in the cells below between 1 and 5 based on the following criteria:

    1 - Strongly Disagree (never)

    2 - Disagree (very rarely)

    3 - Neutral (sometimes)

4 - Agree (most of the time)

5 - Strongly Agree (all the time)

  • This quality-of-life scale has been adapted from Ohio State University's quality-of-life scale and from our own years of experience.

  • Higher numbers in this survey equal a better quality of life. This survey may help you to better visualize the general well-being of your pet. In some cases, even one item on the left-hand side of the chart (for example: pain) may indicate a poor quality of life, even if many of the other items are still positive.
