Welcome to Group Class!

As a small local business, we consider it a privilege to be entrusted with your dogs' training journey and strive to uphold the highest standards of professionalism, integrity, ethical practice, and transparency within the unregulated pet industry.

This form is the official enrollment form for group training. Information about this service may be found by viewing our informational packet: pending updates - check back soon.

Should you choose to enroll, please take care to fill out this form with the intention of allowing us to get to know you and your dog and how we can come alongside you in bettering the life you and your dogs share together. Note that this is a judgement-free zone, and we view working with you as a partnership as a shared effort to meet you where you're at and work with you to get you to where you want to be.

Please note that we have a limit of 2 people per dog present to maintain ample space on the training floor, and all minors under the age of 18 must be accompanied by an adult guardian at all times.

Participant Waivers

The general run down of rules for group class participation.
Due to classes taking place in a group environment, we cannot accept dogs experiencing vomiting, diarrhea, coughing, or parasites (external or internal) into our classes. Participants must keep their dog home if they display any symptoms listed above.*

Payment is due in full prior to the training evaluation. No refunds may be issued for class enrollment following payment. Rollovers to next session may be available on a case-by-case basis, IF requested prior to the *first* date of class for the session and approved by C9C in writing.*

Cloud 9 Canine reserves the right to refer participants to alternate services if our trainer's professional assessment determines that a dog would benefit from specialized training (i.e., advanced behavioral cases requiring intervention from a certified canine behavior consultant).*

Release Waiver: By enrolling in Group Training with Cloud 9 Canine, LLC, I hereby certify that I acknowledge the risks associated with participating in a public group training course and will not indemnify C9C, the host facility, or either parties' associates for any damages or losses related to my participation in a C9C group class or property before, during, and after attendance at the host facility. I hereby release Cloud 9 Canine, LLC from responsibility for any injury or damage to myself, my dog, or property obtained before, during, or after my voluntary attendance and participation in Group Classes at the host facility.*
