Above Reproach Academy Private School Enrollment Form

Private School Enrollment

Submission for enrollment is not complete until required documents are received by Above Reproach Academy. Documents required for each student are 1) Enrollment Form 2) Copy of Birth Certificate 3) Physical Form DH3040 and 4) Immunization Form DH680 or Waiver Form DH Form 681 obtainable at your local Health Department. You will be invoiced for the tuition rate(s) once documentation is received. Above Reproach Academy (DOE# 9632) is registered as a private school in the state of Florida, under Pasco County.
