ONLY A PARENT OR GUARDIAN MAY FILL OUT THIS FORM. EMAIL AND CONTACT INFORMATION MUST MATCH INFINITE CAMPUS RECORDS FOR THE PARENT/GUARDIAN.

Nevada law requires that attendance notes be submitted within 3 days of the student's return. Non-medical absences are limited to 7 per school year. Unverified absences (those without an excuse) may result in denial of credit, retention, or a citation.

This form does not have to be submitted for approved pre-arranged absences. Please see the Student Handbook for more information on attendance requirements.
