1.Prior to beginning the class, Thumb CDL, must have received authorization from the funding institution. 2.Students must present a current US DOT physical examination card and drug screen at the time the course begins.
3. Students must be at least 17 years old to take the classroom portion of this training. Students must be 18 years old to obtain their CLP from the Secretary of State for the behind the wheel portion of this training.
4. Total fee for this course: 5.Payment methods accepted: credit card (3.55% additional convenience fee will be added to the total amount when using a credit card), cash, money order, or checks. Thumb CDL, LLC will not accept postdated checks. Payment plans are available, if needed, and must be pre-approved prior to the beginning of class.
6. Student attendance is required for all class times. In the event of an emergency, our instructor(s) need to be notified as soon as possible. Missed classroom sessions must be made up on the student's time (evenings/weekends Students will be given materials and a quiz that must be completed with an 80% or higher. 7.There is no charge for any materials or supplies provided during the course, however, students will be responsible for the following fees: DOT physical (made payable to the Medical Examiner), Drug Screen & eligibility check (made payable to Thumb CDL, LLC), Commercial Learner's Permit (CLP) made payable to the State of Michigan Secretary of State, and All Road Test Fees (made payable to the tester 8.The student will be charged $75.00 for text books not returned upon completion of the class. 9. The use of Thumb CDL, LLC's tractor-trailer for 3rd party testing is included in tuition cost. However, if a student fails the road test, they will be responsible for the additional road testing fee as well as truck rental. Truck rental may be split amongst multiple students if applicable. Financial help may be available for these costs if needed. 10. By signing this contract, the student agrees to not consume alcohol and/or controlled substances (without prescription) for 24 hours prior to getting behind the wheel of our Commercial Motor Vehicles (CMVs
