    Complete this form with the information of the User who's account you need to update.
  • *Provide the email address you want the updated account to be associated with.

  • Selecting the wrong CHAMP User Role will result in significant delays to accessing CHAMP. 

    If you are not sure which to select follow the instructions below:   

    • Check in with your supervisor on which of the above CHAMP user role to select.  
    • Ask colleague who has a similar job function as yourself  use the image below to help guide where to find a user's role in CHAMP.

  • User's Supervisor Information (U)

  • The role selected requires the user to have a Supervisor linked the account. Please provide the CHAMP ID for this user's supervisor. 

    • To locate the CHAMP User ID 1) Login into the supervisor's CHAMP Account 2) From the home page navigate to "Tools" 3) Select "My User Configuration" then "My Information" 4) You'll see the "User ID" listed.

    • If the user's supervisor is not active or doesn't not yet have a CHAMP account please provide the CHAMP User ID of an alternate supervisor who is currently active in CHAMP. Failure to provide a valid supervisor will results in a rejected request. 


  • New Users must attend training to access CHAMP please answer the following questions to confirm if this user needs training.

    • To locate your user role, login to CHAMP and you'll see your user role displayed on the home page.
    • If you don't know and do not have access to your CHAMP account select "I am not sure."

  • Because the user already has an account, that they've used in the last 12 months and is the same user role type as requested no training is needed.
