Thank you for your desire to be part of our ever-expanding community. We are grateful for you and your support. Please complete the form below to apply to be a mentor for our 2025 program year. Please be prepared to attach your up-to-date bio as well as a recent photo. If you are selected to be a mentor, we will use those assets for our website and other digital promotion purposes.

Please note that for 2025, our Mentor Sign Up Deadline for LEVEL 2 is May 16th. For all program related dates, please refer to our website.

Have you had any previous training related to Diversity, Inclusion and Equality? Briefly note the program and the organizer. Please note that Level 2 Mentors are required to have some kind of DEI training. We provide this training free of cost if you are selected as a Level 2 Mentor.

Please upload a short (1 paragraph) bio highlighting your relevant skills and experience that you'd like associated with your profile, should you be chosen as a mentor. Please write in the third person (e.g. Jane Grapes is a vineyard manager who specializes in...). Please note that this will be shared on our website when you are finalized to participate in the program.
