Curbside Test Instructions:

A. Complete online registration & pay $18.49 test fee

B. Drive to 17000 W North Ave Brookfield, WI

C. Park in WEST PARKING LOT (See image below)

D. Call 262-649-3900 when parked & remain in vehicle

E. Get tested and results emailed within 30 minutes

Mon-Fri 9:30am-5:30pm

Sat 9:30am-12:30pm
