Please note before submitting this form:

- You must be over the age of 18 to be tattooed

- A non-refundable deposit is required to book your appointment, the time slot given to you will not be secured until this has been paid

- Unless an exception is made, I do not show designs or sketches before the tattoo appointment. Please only book in with me if you trust I will do a good job. We can make changes and tweaks to your design on at your appointment if wanted
