I agree to have my hair colored, I understand that Vina's Beauty Bar is licensed. By signing this agreement, I consent to having my hair colored by this licensed professional.

Have you ever had your hair colored before? (Including anything in the last 5 years)
If yes, please explain your hair history with any henna, box dye, color, salon color or any chemicals (such as perms or relaxers used in your hair) provided with dates of the year (if you can’t remember try to remember the best you can!)
Do you have any allergies to any hair color?
A patch tested is recommended if you have had a previous allergic reaction or sensitivity such as (itching, burning, discomfort) to any hair color. Booking this service is to test to see if you will experience any allergic reaction or to see how well your hair lifts to the chemicals, it is required to come 48 hours before your initial appointment. THIS APPOINTMENT IS NOT REQUIRED, Would you like to book this service? (This service is at no additional charge).
Do you understand that if you declined the patch test service, I will not be responsible or liable for any allergic reaction to the service ( such as itching, burning, discomfort) during or after the service. I also understand that if I experience any of these symptoms I will let my hairstylist know.

Do you understand that virgin hair is priced at 90/hr and color corrections are priced at 110/hr
Do you understand that I will do my best to achieve your inspiration picture, if you are going lighter I will need to consider the integrity of your hair first. I will do my best to lighten your hair as light as possible (as much as your hair can handle) but just know that it could take multiple sessions to aim for your inspiration picture.

Do you understand that I am not liable if you experience any hair breakage, also if you weren’t truthful about your hair history and if you weren’t happy about the results afterwards

Do you understand that as a stylist I do my best to preserve your hair’s integrity in the best condition it can be. All color services include K18 treatment, hair masks.
Do you understand (in some circumstances) I will need to cut off _x_ amount of inches to prevent any hair breakage or split ends during your color service.
Do you understand that Vina’s Beauty Bar doesn't offer refunds. Stylist tech cannot take back work, product or time put into the service. Do you agree that you cannot get a refund under any circumstances. Before leaving, I will make sure you leave your service happy/satisfied. Clients must leave the service happy. If any issues do occur within 48hrs from your service, please don't hesitate to get in contact. I am more than happy to fix the issue. Anything over 48 hrs, I will not honor a service to you.

Do you understand if you don’t invest into proper aftercare and follow them, such as color safe shampoo and conditioner, hair mask, leave in conditioner, hair oil, heat/thermal protectant the stylist is not liable for the health of your hair and if your color has stripped/dulled

The agreement will remain in effect for this procedure and all future follow ups conducted by the licensed stylists. I read English and understand that this consent agreement is legal and binding. I have read and fully understand all information in this agreement and hereby release any and all persons representing this salon from all claims, demands, damages, actions and cause of action arising out of ther performance of the service. I have fully disclosed all condtions regarding my health history, medications, treatments and past reactions to products, I consent to the agreement and the hair service.
