Thank you for your interest in your child/children to play baseball with the Memphis Monarchs!

*Attending "Tryouts" WILL NOT guarantee placement on the team your child/children are evaluated for in 2024/2025.

To play Baseball competitively players will need basic skills and a positive attitude including the ability to:

Catch,Throw,Run, Field a ball, Hit, and Move between the bases

The following teams: 8U (ages 7-8), 10U (ages 9-10),12U (ages 11-12), and 14/15U (ages 13-15) will have Tryouts when announced to determine making the team. Each team is limited to a certain number of players determined by the Head Coach. Players may have the option to "play up" from their age group, but they can NOT "play down" from their age group.

T-ball (6U, ages 4-6) will NOT attend "Tryouts". However, all interested players MUST attend Training and Practice Clinics scheduled to be considered by coaches when the team roster is made.

Please realize this is NOT an exclusive recreational program and the focus on competition increases dramatically. The philosophy of this program is to give youth in the Memphis Monarchs area an opportunity to play baseball at an advanced level while preparing them to play at a competitive and eventually High School level.

All teams will have availability for our Monarch Guild consisting of Bat boys/girls, Water boys/girls, and other ancillary team needs for those that do not make final team rosters and are allowed by parent(s)/guardian(s) to do so. Monarch Guild members will also have a "Guild Member" fee broken into 3 levels of participation interested.

By Signing Up Below I/We understand:

- The $20 Registration Fee is Mandatory for "Tryouts & 6U Signups" and does NOT cover any portion of "Player Fees" for the SPRING/FALL 2024/2025 Seasons. The registration fee is Non-Refundable! Payment must be made at Tryout location or prior. Cashapp, Venmo, or Cash available for in person payment.

- I/We understand participation is Mandatory for "Parent Orientation" hosted by Memphis Monarchs Sports Incorporated prior to the 2024/2025 Spring/Fall Season.

- My child/children will be evaluated to determine if he/she will play for Memphis Monarchs Baseball organization.

- I/We understand participation of our child/children is Mandatory for "Player Orientation" hosted by Memphis Monarchs Sports Incorporated prior to the start of the Season.

- I/We the parent(s) of a candidate for a position on a Memphis Monarchs baseball team, hereby give my/our son/daughter approval to his/her participation in any/all activities during the upcoming 2024/2025 seasons.

- I/We assume all risks and hazards incidental to such participation including transportation to and from activities, and I/We do hereby waive, absolve, indemnify and agree to hold harmless the Memphis Monarchs Sports Incorporated, participants and persons transporting my/our child – except to the extent and in the amount covered by liability insurance.

- I/We understand Medical insurance will NOT be provided by Memphis Monarchs Sports Incorporated

-I/We will furnish a certified birth certificate upon request.

*If my child/children makes the Final Roster for a team, the initial 1st installment Payment of "player fees" will be due as determined by Memphis Monarchs Sports Incorporated. I understand that there is a firm "NO REFUNDS" for any money paid to MMSI.

Informed Consent/General Release-Youth Baseball Participants

NOTE: THIS FORM INCLUDES A RELEASE OF LIABILITY.

Since participation in youth sports activities can be dangerous, Memphis Monarchs Sports Incorporated requires that all participants (and their adult parent(s) or guardians) to assume all risks associated with youth baseball by signing this general release.

For and in consideration of my child being permitted to participate in Memphis Monarchs Baseball activities, I hereby voluntarily release, discharge, waive and relinquish any and all claims or actions for damages for personal injury, permanent disability, death, or property damage which I or my child may have, or which may here after accrue to me or my child, as a result of my participation in youth baseball activities during play and while I am at the facility while others play or for any other reason.

This release is intended to discharge, in advance, Memphis Monarchs Sports Incorporated, it’s officers, employees, agents, and the owners and maintainers of any facility used for baseball practice or activities, from any and all liability arising out of or connected in any way with my child’s participation in baseball camps/clinic activities, even though that liability may arise out of negligence or carelessness on the part of Memphis Monarchs Sports Incorporated, its officers, agents or employees, or the owners or maintainers of any facility used by Memphis Monarchs Sports Incorporated for baseball practice or activities. I further understand that serious accidents occasionally occur during youth baseball activities, and that participants occasionally sustain serious personal injuries, death or property damage as a consequence thereof. Knowing the risks, I have voluntarily applied for my child to participate in the activity and thereby agree to assume those risks to release and hold harmless Memphis Monarchs Sports Incorporated, its officers, employees or agents, or the owners or maintainers of any facility used by the Memphis Monarchs for baseball practice or activities, who (through negligence or carelessness) might otherwise be liable to me or to my child (or my heirs or assigns) for damages.

I further understand and agree that this release, discharge, waiver, and assumption of risk is to be binding on my and my child’s heirs, executors, administrators, and assigns. I further agree to indemnify and to hold harmless Memphis Monarchs Sports Incorporated, its officers, employees and agents, or the owners or maintainers of any facility used by the Memphis Monarchs for baseball practices or activities, for any loss, liability damage, cost or expense which may incur as a result of any injury or property damage I or my child may sustain while participating in the activity. I agree to comply with the program’s stated and customary terms and conditions for participation according to Memphis Monarchs Sports Incorporated.

**If I observe any significant change with regards to my child’s readiness for participation in the program, I will remove my child from the program immediately. I understand that once any or all installments of "player fees" have been made that there are absolutely NO REFUNDS.

I have read this Informed Consent/General Release, fully understanding its terms, that I give up substantial rights by signing it, and sign it voluntarily.
