Under the Data Provision Requirements 2012, ISMT is required to collect personal information about you and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER). Your personal information (including the personal information contained on this enrolment form and your training activity data) may be used or disclosed by ISMT for statistical, regulatory and research purposes. ISMT may disclose your personal information for these purposes to third parties, including:
• Commonwealth and State or Territory government departments and authorised agencies;
• NCVER;
• Organisations conducting student surveys; and
• Researchers.
Personal information disclosed to NCVER may be used or disclosed for the following purposes:
• Issuing a VET Statement of Attainment or VET Qualification, and populating Authenticated VET Transcripts;
• facilitating statistics and research relating to education, including surveys;
• understanding how the VET market operates, for policy, workforce planning and consumer information; and
• administering VET, including program administration, regulation, monitoring and evaluation.
You may receive an NCVER student survey which may be administered by an NCVER employee, agent or third party contractor. You may opt out of the survey at the time of being contacted.
Student Declaration and Consent
1. I understand and agree that the course fee is non-refundable if I do not attend the course within the specified timeframe.
2. I am over 18 years old at the time of signing this agreement or, if under 18 years old, my parent or legal guardian has signed on my behalf
3. I understand the fees that must be paid to undertake this course, including additional fees that may apply during the course
4. I understand that the course must be completed within one week from the start of the practical component. If I do not finish within this timeframe, I will be considered not competent and will need to re-enroll in the next available intake.
5. I confirm that I have received at least two doses of a COVID-19 vaccine recognised by the Therapeutic Goods Administration (TGA) and will provide evidence of this prior to entry to any campus
6. I authorise ISMT to create a USI for me if I do not already have one and to use information provided by me to verify or create a USI
7. I understand that ISMT may cancel, withdraw or delay the course if there are not sufficient enrolments.
8. I understand that I will use my own IT resources such as laptop, computer or tablet for training and assessment purposes.
