Community Wellness Registration Form
Are you a San Diego based artist, educator, healthcare worker or community organizer? We have an 8-week program where we offer services at a lower cost to support individuals who are contributing to the enrichment of the San Diego Community. Please fill out this registration form to join our community wellness program.

Community Rate

$55 - $75 for one hour session

CWP Appointments Available

Saturday 9:30-10:30am (Starting October)

Saturday 12:30-1:30pm

Saturday 1:30-2:30pm

$440 for 8 one hour sessions ($55 per session) - Option 1 - You are a student or artist, do not own a home, you share rent with roommates, income is not consistent$520 for 8 one hour sessions ($65 per session) - Option 2 - You do not own a home, rent an apartment, have a partner and/or roommate, income is consistent$600 for 8 one hour sessions ($75 per session) - Option 3 - You own a home, have consistent income, and/or have combined household income
Upon completing our community wellness program registration you will be emailed within the week to confirm a scheduled appointment time. You will also be sent an invoice to pay a deposit to secure your first appointment. I look forward to working with you!
