    I am aware of the risks in observing or participating in the activities offered and sponsored by CBT GYM and I understand that all sports or fitness that I will execute and participate in are entirely at my own risk and perils. I assume complete responsibility and liability for those risks and for the injuries that may occur as a result of these risks, even if injuries occur in a manor that is not forseeable at the time I sign this agreement. I realize that by voluntarily assuming the risks involved, I will be solely responsible for any loss or damage I sustain, including personal injuries to me, damage to my property, or damage arising out of my death. It is understood that CBT GYM is not responsible for any damage that would be resulting from a fault of CBT GYM and its representatives.

  • Code of Conduct

    Members must adhere to the following "Code of Conduct". Any members found to violate any aspect of the CBT GYM "Code of Conduct" may face immediate suspension or cancellation of their membership; A permanent ban from the CBT GYM facility and/ or products and services provided by CBT GYM and/or its representatives, may be applicable.
  • 1) CBT GYM takes the safety and security of all members and staff very seriously. There is a ZERO TOLERANCE policy for violence, bullying and harrassment of any kind. We ask for everyone to respect fellow members, staff, visitors and other community members. Always keep safety top of mind when using the CBT GYM facilities. If you see something that may be unsafe, whether it is gym equipment, the building itself, or the conduct of another member, please don't hesitate to advise a CBT GYM Staff member or Representative.

     

    2) While we do our best to maintain a clean facility, we appreciate members continuing to use the disinfectant spray and paper towels provided throughout to wipe down any machines or equipment they may use. Indoor shoes are always required in order to keep the floors clean and safe. Should you spill or make a mess of some kind, please try to clean it up or let a staff member know if you require assistance (broom, mop etc.)

     

    3) Be Courteous. Please always re-rack your weights - whether free-weights or plates. There are many members who cannot lift some of the heavier free-weights or plates as well as those who have physical limitations that make it difficult to do so. Be respectful and always re-rack your weights.

     

    4) We are happy to provide access to the CBT GYM facility 24/7 through the use of Key Fobs. Membership status is only known, and can only be confirmed and verified by CBT GYM Staff and Authorized Representatives, therefore members are PROHIBITED from allowing anyone to enter the gym without the use of their own fob. This means that you may not open the door to the gym for someone who has "forgotten" or does not have a Key Fob on their person, regardless of whether they intend to use the facility or not. This includes friends, family, colleagues and spouses etc. Should someone have a question, concern or inquiry, please direct them to contact a staff member (contact information is provided on the door.) Failure to adhere to this rule, will result in immediate termination of your membership - no refunds will be provided.

  • Billing Policy

    I authorize CBT GYM to charge my payment card (provided by me upon sign up) on the date and for the amount agreed upon at the time of sign up; for monthly memberships this includes regular billing of my card on the 1st of every month. I understand that this agreement is withstanding until the time I have requested to cancel my membership with CBT GYM. I acknowledge and understand that it is my responsibility to ensure that the payment information associated with my CBT GYM Membership is current and active to avoid any missed or declined payments; I agree to update my payment information with CBT GYM within 2 business days of any change or cancellation of the payment information I had previously provided for regular billing of my CBT GYM membership; this includes but is not limited to cancellation, reissue or expiry of the payment card or account I provided. I agree to pay any overdue amount within 7 business days of a missed, declined or other overdue payment and that additional fees may be applicable. I realize that CBT GYM implements the following sub-policies surrounding memberships and billing:
  • Cancellation - 
    Should any member wish to cancel their CBT GYM membership, the request for cancellation must be received in writing to cbtmemberships@gmail.com or by letter delivered to a CBT GYM representative, no less than 2 weeks prior to the 1st of the month in which they wish to cancel. Please note, if cancelling a monthly membership, the membership will be valid until the final day of the month following the month of cancellation, as members pay first and last month's fees up front (Example: Cancellation received November 17, membership valid through end of day December 31.) The request must be received from the member or their authorized representative. Cancellation emails will be responded to, acknowledging the request, and will be processed within 2-3 business days. If the member is cancelling an annual membership, please note that the membership will remain valid until the date of expiry (date of purchase in following year - example purchased November 17, 2023 - Expiry = November 17, 2024) regardless of request for cancellation. No refunds will be issued for cancelled annual memberships, or for monthly memberships cancelled outside of the required notification period.

    I have read and understand the cancellation policy:

     

  • Re-Activation - 
    Upon reactivation of a previously cancelled membership, the member will be subject to a $69 re-activation fee in addition to our standard charges (for monthly membership this includes first and last month's membership fee with first month being pro-rated based on date of sign up. For annual membership, this includes payment in full for the entire year) as indicated and agreed upon by the member at the time of sign up. If the member does not have their existing Key Fob at the time of re-activation, we will be required to provide a new Key Fob, for which there will be a fee of $25. 
    If the member reactivating had previously unpaid invoices for membership, plan or other products or services provided by CBT GYM or its representatives, these amounts will be required to be paid in full and an NSF ($29) or other administrative fees may be applicable. 
    Members who have cancelled and would like to reactivate their membership will also be subject to any price increases that may have occurred.

    I have read and understand the Re-Activation Policy:

     

  • Declined or Unsuccessful Payment - 
    Late/Declined Payment - full policy
    As of September 1, 2023, ALL declined or unsuccessful monthly membership payments will be subject to a $29 fee. This will be billed to the member for any missed or declined payments whether the payment was unsuccessful for any reason, including (but not limited to) Non-Sufficient Funds, Expired payment information etc. 
    EFFECTIVE JULY 1, 2024 - 
    CBT GYM and/or one of its authorized representatives, will attempt to contact members regarding any declined or unsuccessful payments, via email and/or telephone to the contact information provided at sign up. If the member does not respond to any contact received from CBT GYM and/or one of its authorized representatives or fails to contact CBT GYM within 5 days of a declined or unsuccessful payment, the member's Key Fob will be deactivated. If an overdue balance exceeds 5 days, the “last month” fee initially paid at the time of sign up will be forfeited by the member, to be used by CBT Gym in lieu of the applicable Late Payment Fee. At this point the membership will be cancelled immediately. To resume membership with CBT GYM following cancellation, the member will be subject to the Re-Activation Policy. 

     

  • Refunds - 
    CBT GYM has a firm NO REFUNDS policy for any membership or other plan, product or service purchased from CBT GYM or its representatives. It is your responsibility to provide sufficient notice to cancel your membership (details above); Should you fail to pause or cancel your membership, you will not be refunded for any period of time you did not or will not attend the gym. If you feel there has been an error in billing, please advise a CBT staff member and it will be investigated, but there is no guarantee of a refund. 

    CBT GYM memberships, plans or packages can not be transferred to another person or member. Monthly members may cancel their membership (as detailed above); annual memberships will remain valid through their expiry date for the original purchaser/member only, no refunds will be issued if "unused". 

    **PLEASE NOTE: Memberships must be used by the original purchaser/contract holder only. They may not be shared, transferred, traded, given or sold to another person**

  • I certify that I have read, acknowledge and understand all of the information and policies explained in this agreement. I also certify that any and all information I have provided is true and correct. I agree to follow the policies and conduct outlined throughout this agreement. I understand that the information I have provided, and that has been provided to me in this agreement is private and confidential and will only be used for internal purposes by CBT GYM and its authorized representatives, and will not be shared. I acknowledge and accept that this agreement will be retained by CBT GYM, even in the event that I cancel my membership.
