Chargeback Penalty: In the event of a charge back, all payments submitted to center will be forfeited to Body By Doctor. For those individuals who pay by credit card, debit card, or finance companies: you are not eligible for credit card challenge or "charge back" to the finance companies once the service is provided as per this Agreement. You hereby understand and agree that (1) dissatisfaction with a procedure due to personal taste or otherwise, or (2) cancellation of a procedure under this Agreement are not grounds to initiate a charge back with your credit card, debit card, or finance company. Patients who fraudulently initiate charge back transactions and/or fraudulently dispute charges or services will be prosecuted to the fullest extent of the law. In addition, a $100 fee will be assessed.

Body By Doctor Booking Agreement:-
WARNING: THIS IS A LEGALLY BINDING CONTRACT. BODY BY DOCTOR IS A THIRD-PARTY BOOKING AGENCY, NOT A MEDICAL PROVIDER. ALL PAYMENTS ARE FINAL AND NON-REFUNDABLE UPON BOOKING CONFIRMATION, VERIFIED BY YOUR IDENTITY DOCUMENTS AND SIGNATURE. FRAUDULENT REFUND ATTEMPTS WILL FACE SEVERE PENALTIES AND LEGAL ACTION.
By signing, you irrevocably agree that you have read, understood, and are bound by all terms. Booking services are deemed fully provided upon verification of your identity and scheduling of your surgery time slot.
1. Role of Body By Doctor
1.1 Body By Doctor is a third-party booking agency that facilitates the scheduling, rescheduling, and coordination of surgical procedures with independent surgeons and facilities.
1.2 We do not provide medical services, perform surgeries, or manage surgical outcomes or post-surgery care. Our service is limited to securing your surgery time slot and coordinating with your selected surgeon’s office.
1.3 Booking is the service: By submitting your identity verification and signing this agreement, you acknowledge that Body By Doctor’s service is complete upon confirmation of your surgery booking, regardless of whether you attend the surgery, are satisfied with the surgeon, or meet the surgeon’s requirements.
2. Identity Verification for Service Delivery
2.1 To confirm your booking and secure a surgery time slot, you must provide the following, which serve as proof that Body By Doctor has delivered its booking services:
2.2 A clear, legible copy of your government-issued photo ID (front and back).
2.3 A recent selfie clearly showing your face, matching the photo ID.
2.4 Your signature on this agreement, verifying your identity and consent to all terms.
2.5 Failure to submit these items will void your booking, resulting in immediate cancellation and forfeiture of all payments, with no refund, exchange, or transfer.
2.6 Service Proof: Submission of these items and receipt of a booking confirmation (via email, text, or coordinator communication) constitutes irrefutable evidence that Body By Doctor has provided its booking services in full.
3. Booking and Lab Requirements
3.1 Labs: You must complete labs between 8:00 AM and 5:00 PM, the day before or day of surgery, as coordinated through your booking.
3.2 Fasting: You must fast for labs and surgery per the surgeon’s instructions. Non-compliance may lead to surgeon cancellation, with no refund from Body By Doctor.
3.3 COVID-19 Testing: Valid COVID-19 test results must be presented at labs or on surgery day, as required by the surgical facility.
3.4 Booking Deposit: A non-refundable deposit is required to secure your surgery time slot. This deposit compensates Body By Doctor for booking, coordination, and administrative services. Deposits are non-refundable, non-exchangeable, and non-transferable under ALL circumstances, including but not limited to cancellation, rescheduling, dissatisfaction, failure to attend, or surgeon refusal to proceed.
4. Rescheduling Policy
4.1 Notification: Rescheduling requires notifying your coordinator at least seven (7) days before your booked surgery date.
4.2 Fees:
• Before surgery: $300 coordination fee.
• After surgery date or late notification: $400 fee, plus a new non-refundable deposit.
4.3 Non-Compliance: Failure to notify in time results in booking cancellation and forfeiture of all payments. No exceptions.
5. Booking Confirmation
5.1 The surgeon’s office will contact you to confirm your surgery time.
5.2 Failure to confirm with the surgeon will cancel your booking, forfeiting all payments, including your deposit. Body By Doctor is not liable for communication failures between you and the surgeon.
6. Procedure Modifications
6.1 Mandatory Process: To add or remove procedures, contact your coordinator first, even at the surgical facility. Unauthorized changes are invalid.
6.2 Fee: $200 per modification.
6.3 Limitations: Requests less than seven days before surgery are not guaranteed. Body By Doctor is not liable for surgeon denials.
7. Media Consent
7.1 You grant Body By Doctor irrevocable permission to use photographs, videotapes, and/or case histories provided during booking for verification, coordination, publicity, or certification purposes. Body By Doctor will protect your privacy as required by law.
8. Payment Terms
8.1 Surgeon Payments: Surgeons accept only cash for their services, as coordinated through your booking. Body By Doctor does not handle these payments.
8.2 Booking Fees: All Body By Doctor fees (deposits, rescheduling, modifications) are in U.S. Dollars (USD) and non-refundable.
8.3 Rebooking: A new non-refundable deposit is required to rebook with the same surgeon.
8.4 Service Completion: Booking services are complete upon identity verification and scheduling confirmation. No refunds or disputes are permitted once services are provided, regardless of surgical outcome or attendance.
9. Post-Surgery Responsibilities
9.1 Body By Doctor is not responsible for surgical outcomes, complications, or post-surgery care, which are solely the responsibility of the surgeon and their team.
9.2 You must obtain all necessary post-surgery contact information from the surgeon before leaving the facility. Body By Doctor will not assist with post-surgery matters.
10. Cancellation Policy
10.1 Body By Doctor may cancel your booking at its sole discretion for reasons including:
10.2 Failure to provide required identity verification.
10.3 Aggressive, abusive, or threatening behavior (e.g., personal attacks, abusive language, threats, physical contact, aggressive gestures).
10.4 Non-compliance with any term of this agreement.
10.5 Cancellation forfeits ALL payments with no refund, exchange, or transfer.
10.6 Zero-Tolerance Policy: Body By Doctor enforces strict consequences for misconduct or non-compliance.
11. Anti-Refund Abuse and Chargeback Prohibition
11.1 ALL PAYMENTS ARE FINAL. Body By Doctor’s booking services are fully provided upon verification of your identity (ID, selfie, signature) and confirmation of your surgery time slot, as evidenced by our records.
11.2 Claiming non-delivery of services to seek a refund or chargeback is fraudulent and will be treated as such, given that booking services are complete upon scheduling confirmation.
11.3 Initiating a chargeback, dispute, or refund request for any reason—including dissatisfaction with the surgeon, failure to attend, cancellation, or personal preference—is strictly prohibited.
11.4 Consequences of Refund Abuse or Chargebacks:
11.5 Immediate forfeiture of ALL payments made to Body By Doctor.
11.6 A $150 penalty fee, due immediately.
11.7 Legal action to the fullest extent of the law, including prosecution for fraud and recovery of all associated costs.
11.8 You waive all rights to dispute charges once booking services are provided, as verified by your identity documents and booking confirmation.
11.9 Body By Doctor maintains detailed records of your verification and booking to defend against fraudulent claims.
