  • To rent a catering van, you must first contact the General Manager and/or the Assistant Manager to verify that the van is available for the time that you want to use it.

     

    Once this has been done, you may complete this form to officially secure the van.

  • SECTION 1. RENTER INFORMATION - This agreement is made and entered on this hereinaftercalled of , 20 between hereinafter called "RENTER". as agent for

    SECTION 2. RENTAL VEHICLE - THE OWNER represents that to the best of the OWNER's knowledge and belief that said vehicle is in sound and safe condition and free of any known faults or defects which would affect its safe operation under normal use.

    SECTION 3. RENTAL PERIOD -The OWNER agrees to rent the van between the times listed above. 

    SECTION 4. VEHICLE RENTAL RATE - The RENTER hereby agrees to pay the OWNER at the daily rate of $20.00 or 1/2 day rate of $10.00 for the use of said vehicle.

    SECTION 5 AUTHORIZED VEHICLE DRIVERS - The RENTER certifies that the below listed persons are fully qualified and posses a valid U.S. drivers license and will operate the vehicle as auxiliary driver(s) during the rental period. (Note: Only employees whose names appear on this rental agreement with a current signed Driving History Questionnaire on file are authorized to operate this vehicle

    SECTION 6. CONDITIONS & RESTRICTIONS -The RENTER further agrees to abide by all conditions and restrictions in this agreement.

    Restrictions The RENTER understands that the rental vehicle is a TOBACCO FREE vehicle and in the event that evidence of tobacco use of any kind is detected, as determined by the OWNER, the RENTER is subject to loss of rental privileges and the cost to clean the vehicle.

     

  • The RENTER agrees to operate the vehicle in a safe and prudent manner, particularly as regards to but not limited to the following considerations:

    1. Any driver of the vehicle shall at all times be in possession of a valid United States driver's license.
    2. Non-authorized use of the vehicle is prohibited.
    3. The number of passengers may not exceed the number of seat belts.
    4. The driver assumes full responsibility for any fines resulting from traffic or parking violations arising out of the use of the vehicle.
    5. Transporting hitchhikers or other non-authorized passengers is not permitted at any time.
    6. No smoking is allowed in the vehicle.
    7. The vehicle ignition must be off and locked when unattended.
    8. Driving is not permitted while under the influence of alcohol or drugs.
    9. Drivers and all passengers are required to wear seat belts when the vehicle is moving.
    10. Consideration of turning off the passenger side airbag shall only be made when a child is riding in the passenger front seat, and only after reading the OWNER'S manual for compliance requirements.
    11. Fully loaded vans shall not be operated over 75 miles per hour.
    12. For safety reasons, the vehicle shall not be used to push, propel, or tow another vehicle(s For safety reasons, installation of any and all luggage racks or other items on the vehicle roof is prohibited.
    13. Because of driver fatigue, drivers shall not drive more than four hours continuously without stopping for rest, nor exceed more than fifteen hours driving in any twenty-four hour period.
    14. An awake alternate driver is required for any and all late-night driving.
    15. In the event of damage to the vehicle the RENTER is liable for the $500.00 deductible not covered by the OWNER'S vehicle insurance policy.
    16. In the event of damage to the vehicle and any other vehicles due to collision it is the driver's responsibility to notify the OWNER of the accident in a timely manner and obtain a copy of the
    17. The vehicle shall not be used to carry passengers or property for hire.
    18. The vehicle shall not be used for carrying passengers other than in the interior or cab of the vehicle.
    19. The vehicle shall not be used for any race or competition.
    20. The vehicle shall not be used for any illegal purpose.
    21. Authorized drivers shall not operate said vehicle in a negligent manner. 
    22. Cell phone use while driving should be kept to a minimum. Drivers need to be aware when use of the cell phone is creating a distraction from safe driving and adjust their usage accordingly, including pulling off the road to continue/finish the conversation if needed. Whenever possible, Drivers should complete calls while the vehicle is parked and/or use the phone in a "hands-free" mode via a headset or speaker. While driving, attention to the road and safety should always take precedence over conducting business over the phone.
