Once you have completed all sections and submitted the form, you will then be taken to the payment screen.

1. Eligibility

• The offer is open to individuals who are 18 years of age or older.
• Participants must be free from any dental disease.
• This offer is subject to any applicable laws and regulations and is void where prohibited.

2. Oral Health Assessment

An oral health assessment will be conducted by our clinicians during the initial visit.
The suitability for the Enlighten Evo4 Whitening treatment will be determined based on this assessment.

Patients who have existing dental restorations such as crowns, bridges, dentures, veneers or fillings, please note these restorations will not respond to whitening treatments. 

Should any dental x-rays be required to confirm your dental health, this will be an additional charge of £10 per x-ray.

3. Refunds

• If a participant is deemed unsuitable for the treatment, they will be entitled to a full refund.
• The refund process will be initiated immediately upon determination of ineligibility, and the amount will be credited to the original mode of payment within 7 business days.

4. Booking and Cancellation Policy

Appointments must be scheduled in advance and are subject to availability.
We request that participants provide 24 hours notice for any cancellations or rescheduling.
Failure to provide timely notice may result in forfeiture of the offer or a cancellation fee.

5. Liability

The clinic is not liable for any adverse reactions or side effects arising from the Enlighten Evo4 Whitening treatment, provided that the treatment is administered according to the standard protocol and all patient disclosures are accurate and complete.

6. Data Protection and Privacy

Personal data collected during the booking and treatment process will be handled in accordance with our privacy policy, ensuring confidentiality and compliance with data protection laws.

7. Changes to the Offer

The clinic reserves the right to modify or cancel the offer at any time without prior notice. However, any changes will not affect bookings already made.

8. Acceptance of Terms

Participation in the offer constitutes the participant's full and unconditional agreement to these Terms and Conditions.
