| 1) I undertake to adhere to the Academy Policy and Code of Conduct. |

| 2) I undertake to recognize the authority and responsibility of the academy to maintain discipline (legally teachers are in loco parentis) and accept that on occasion it may be necessary that this may include allowable disciplinary measures against my child. |

| 3) I undertake to allow my child to my own mode of transport as and when needed and indemnify the academy and the staff against liability in this regard. DSQA does not over offer transportation of learners and this service must be arranged by parents/guardians with third parties. DSQA has a few contact details of drivers we can share with parents, DSQA is not however associated with these private drivers. |

| 4) I accept that my child will take part in extra-mural activities and sport of this academy. |

| 5) I accept that in the case of an emergency the academy will endeavor to contact the parents/guardian, failing which the academy will contact the doctor listed above. Any costs will be for the account of the parent/guardian and not DSQA. |

| 6) I duly authorize Darus Saulieghien Quraan Academy (DSQA) to obtain any information pertaining to my child from a previous school/s or institution which my child has attended. |

| 7) I, as a parent/guardian of the abovementioned learner, acknowledge that I am familiar with the admission requirements of Darus Saulieghien Quraan Academy (DSQA) which includes the compulsory payment of tuition fees as determined by Darus Saulieghien Quraan Academy (DSQA). |

| 8) Academy policy allows learners to participate in activities outside of the classroom at extra costs to the parent/s or guardian/s such as grade-appropriate learning activities, outings, excursions, or tours. In these cases, parents must have the academy fees up to date for their children to participate. I hereby give consent for my child to partake in outings and excursions throughout the year to various places and to take part in extramural activities for the current academic year. I fully understand and accept that all the activities shall be participated in and undertaken at my child’s own risk. I hereby indemnify, hold harmless and absolve the staff, other members of the Darus Saulieghien Quraan Academy (DSQA) including accompanying parents against any or all claims that may arise in connection with the loss of or damage to the property of or injury to my child during the course of activities on the outing, in the knowledge that the manager will nevertheless take all responsible precautions for the safety and welfare of my child. I undertake to give the persons in charge my full co-operation throughout the trip. |

| 9) I permit Darus Saulieghien Quraan Academy (DSQA) to do a credit check on myself. |

| 10) I hereby declare that all information contained in this application is accurate in every manner. Any miss information may result in this application being revoked. |

| 11) Fees are to be paid monthly and are due pro-rata in advance before the start of the upcoming month. For example, February (up-coming month) fees are due from 25 January (prior month) and before the1st of the upcoming month, and so forth. |
