A representative from WCC/The HUB will contact you weekly, on Wednesdays, to confirm your student listed above has your permission to ride in a Wildwood Chapel vehicle, to Adventure Club, that week. Unless the WCC/The HUB representative is able to contact you, to confirm permission, your student will not be permitted to ride the Wildwood Chapel vehicle that week.
I, the parent or legal guardian of the student listed above give permission for the above listed student to be transported from WCC/The HUB or Union School by an approved representative of Wildwood Chapel, in a church vehicle, for Wednesday evening activities at the church during the 2025-2026 school year. I release Wildwood Chapel and The HUB (Wyandot Community Connections, Inc) with all representatives, from any and all claims resulting from injury or damage that may be sustained by my child while participating in travel to and from Adventure Club Ministry and The HUB/HUB Kids (a program of Wyandot Community Connections).
