  • Performing regular Glucose Curves on your diabetic pet is essential to proper management. Our team of doctors are glad to perform these in our office, but some pets will do better at home if your able and comfortable in doing so.

    To do so we require clients to use the form below to submit current history and results of in home glucos curves. This is the only form we accept to standardize the information in our records. Please do not use other forms to report values to us. Once submited the form is emailed to us, and uploaded to your pets records. 

    There is a nominal fee for our docotors time to review these results and communicate with you. After any applicable fees are collected and a doctor has the oppurtunity to review the results, we will contact via your prefered method you desiganted on the form.

  • INSTRUCTIONS

    1. Perform an 8-to-12-hour curve 2 weeks after any change in dose or as instructed by your Veterinarian.
    2. Give your pet their AM meal and Insulin dose as normal. Patient MUST eat their normal meal the morning of the curve. Do not change insulin dose without consulting with your pet’s veterinarian first.
    3. Take a First blood glucose reading after insulin administration, record below.
    4. Sample and record the blood glucose level every 2 hours for at least 8 hours. Preferably, continue sampling until the next dose of insulin. 
    5. Submit the completed form when completed.  The form uploads auromaticaly to our Email once submited.  
    6. Follow the payment link after you submitted the form, once payment is confirmed, the results will be reviewed by a veternarian, and we will reach out with instuctions.

  • Warning!

    If a pharmacist has recomended or changed your pet's inuslin to Humilin you need to be aware it tends not to be as effective as the Novilin Insulin.  

    Though it is cheaper, higher doses may be needed to regulate your pets glucose levels.  This may yeild more cost related to more curves to adjust the insulin dose, or more insulin being needed.  

    Please consult with us before you ever allow a pharmacist to change the medicaiton we have prescriped.  Human pharmacist often do not have the training to make appropriate changes to veterinary medicaitons.
