Please fill out and submit this form to become a Caregiver Coffee Club Member. Memberships will be renewed on a quarterly basis to meet our participation reporting requirements. To be eligible for this service, your child or youth must have a diagnosis of Autism Spectrum Disorder (ASD) and be registered in the Ontario Autism Program (OAP). Before you proceed please ensure you have your OAP Registration Number handy.
