Collaborative Effort:

Developing a customized weight loss program requires significant time, expense and effort from our team. By filling out this form, the hotel expresses their intention to work collaboratively with us, understanding that this process involves dedication from both sides.

Guest-Centric Approach:

There are no direct costs to the hotel for offering this program. Guests will cover the program fees, which are added on top of their stay costs. 

 

This is not a Legally Binding Agreement: 

This is simply and acknowledgement we are moving forwards with the program to be offered in your hotel, and respect each others time. We trust that the hotel will see the mutual benefits in this arrangement and commit to supporting the program’s success.

For the following 7, 14 and 21 night packages, please include the tottal cost for what you would like to charge, including:
