Note: In complex or higher risk cases, patients may be referred to a primary care or urgent care for further evaluation. We will make every attempt to notify patients we know will be excluded from our treatment criteria prior to their appointment time. Filling out the following form as completely as possible will aid greatly in our preparation for your appointment.

Additionally, please note, due to lack of federal recognition of Pharmacists as providers we are unable to bill Medicare for clinical services at Vashon Pharmacy.

 

NOTE: Because pharmacists are not recognized as providers at the federal level we are unable to bill Medicare for medical services. If you have a medicare insurance plan we will unfortuately not be able to bill insurance (likely cost of appointment is up to $150). 

 

Insurances currently accepted for Clinical Services (this is medical insurance, not billed through the pharmacy coverage):

• Aetna
• Kaiser
• Molina
• Cigna
• Ambetter
• United Healthcare

 We continue to seek contracts with other insurance providers, if your insurance provider is not listed above, we can still treat you and you will be billed for an out-of-pocket fee for service ($150).
