ONE LIFE I LIVE IT - INTERNATIONAL ICE BATH DAY 2025

I (as named above (Participant have willingly enrolled in this breathwork and ice bath session(s) (Activity) and do so at my own risk.

1. I understand that the Coaches are not clinically trained ie. are not Psychiatrists, Psychologists or Doctors, and cannot prescribe medication nor any form of medical treatment. I understand that in regards to the Breathwork and Cold- Water Immersion, Mark Hughes is a certified XPT Breathing Coach, Apnea Australia Breath-hold for Survival Instructor, Oxygen Advantage Coach, Ice Bath Coach and has extensive experience in coaching and facilitation.

2. As a Participant, I understand and agree that | amfully responsibleformy wellbeingduring the sessions, including my decisions and choices. I am aware that I can choose to discontinue the sessions at any time.

3. I understand that Breathwork & Cold-Water Immersion may have positive effects on mental health issues but understand that the organisers and coaches do not make any claims that these modalities will treat mental disorders as defined by the Australian Psychiatric Association. I understand this is not substitute for substance abuse treatment or other chronic mental health treatment or pharmacological treatment.

4. I understand that | might find aspects of the breathwork and cold immersion, physically and mentally stressful and understand they can trigger emotionally strong responses.

5. I understand that cold water immersion and breathwork can be taxing on the body and mind. I acknowledge that I do not have to participate in any of these sessions and can stop at any time of my own accord but in doing so, I do willingly and in the knowledge that I do not have any medical condition that would prevent me from doing so. | have been advised to consult my medical practitioner and/or psychotherapist about my physical and mental ability to safely participate in the activity. Such medical conditions I can confirm as below and may not be limited to those listed below. I understand it is my sole responsibility to access the activities that are safe for my current level of health and should my circumstances change, I will advise the facilitator immediately.

6. I understand that the information is held confidentially unless stated otherwise in writing, except as required by law. I understand that should I feel suicidal or fear for my own wellbeing I should contact the emergency services on 000 or an organisation such at Beyond Blue 1800 512 348 or Lifeline Australia 13 11 14 immediately.

7. In consideration of the activities, I hereby waive, release, and indemnify and hold harmless Mark Hughes, One Life Live It and any other Directors, organisers, officers, employees and agents from all claims, costs, liability and expenses for injury, loss or damage arising from my participation in breathwork and cold exposure.

8. This Waiver & Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I acknowledge I have thoroughly read this Waiver & release of Liability in its entirety and fully understand and agree to it. By signing this document, I am waiving certain rights I and/or my successors might have to bring legal action or assert a claim against Mark Hughes or One Life Live It and/or any assigns of beneficiaries.

9. I understand that during the event, there will be still photography and video being taken for use in marketing materials including social media. I also understand that independent media outlets will be covering the event via press, radio, social media, and television. In signing below, I expressly give permission for any footage and coverage to be used for all purposes relating to the event or in the future marketing of One Life Live It activities.
