    The following Optimal Health Survey 2.0 is a self assessment tool to gauge fundamental healthy lifestyle habits.  For each question you will rate your level of dedication and commitment on a scale of 1 to 5 with 1 = "I don't do a good job with this aspect of my lifestyle" and 5 = "I do an excellent job with this aspect of my lifestyle". At the completion of the Optimal Health Survey 2.0 you will receive your Optimal Health Score.
