I voluntarily consent to undergo the spray tanning service provided by Orah Sunless Inc. I understand that this service involves the application of a cosmetic tanning solution to the skin.

I acknowledge that potential risks may exist, including but not limited to skin irritation or allergic reactions. I confirm that I have disclosed all relevant medical conditions, allergies, and skin sensitivities to the service provider prior to treatment.

I agree to follow all pre-treatment and aftercare instructions as provided by the professional. I understand that failure to follow proper pre-treatment preparation may affect the development and final outcome of the tan.

I understand that results may vary based on individual skin characteristics and that the depth, tone, and longevity of the tan cannot be guaranteed.

I hereby release Orah Sunless Inc., its owners, employees, and contractors from any liability associated with this service.
