• Please list the first names, last names and relation to you of each family member included on your membership.

    You count as 1 person on your membership. Therefore, the total number of family members listed below, plus yourself, cannot exceed the membership option you selected above.
  • Please read the following terms carefully. A copy of this agreement will be sent to your email address.

    Contract Duration and Automatic Renewal
    Contract is in force for monthly renewal from the Contract Start Date until canceled by either Client or Fitness Alive LLC. with written notification of 30 days in advance of the next billing date. Cancellation notifications can be sent to info@fitnessalivephilly.com or to the program manager listed at fitnessalivephilly.com. 

    Contract Inclusions 
    Client may attend “Family Swim Membership” sessions as part of the Family Swim Membership. Membership includes Client as well as family members listed below in section (c).

    Agreement to Pay Recurring Fees
    Client agrees to pay Fitness Alive LLC. monthly recurring fees as follows: Family Membership Auto-Pay at the rate indicated above via client-authorized automatic credit card. Monthly payments will run on the day of the month corresponding with this contract start date.

    (a) I hereby certify that I am the holder of the credit card.

    (b) I hereby certify that I am aged 18 or older at the commencement of the contract.

    (c) In addition to myself, I certify that my membership includes ONLY the individuals expressly listed above. 

    (d) I understand that I will be notified if my credit card payment fails to authorize for any reason, and that a $10 late fee will apply if I do not provide valid credit card information within 10 calendar days of the original rejection date.

    (e) I understand that Fitness Alive LLC will terminate my membership if full payment, including applicable late fees, is not received by the 11th day following the monthly renewal date.
