INSTRUCTIONS
- This form is designed to facilitate a discussion between the manager and team member and should be filled out electronically.
- The discussion must be conducted at least once annually at year end.
- Performance should be discussed and ongoing feedback provided on a regular basis throughout the year.
Step 1: Associates complete the following:
- Complete Team Member rating in section 1. CORE VALUES and Team Member Comments:
- Enter objectives from section 5 into section 2. YEAR END PERFORMANCE REVIEW and complete Team Member Rating and Team Member Comments:
- Complete section 3. YEAR END PERFORMANCE DISCUSSION questions 3 & 4
- Complete section 4. YEAR END INDIVIDUAL DEVELOPMENT ACTIVITIES
- Develop and enter Objectives in section 5. UPCOMING YEAR PERFORMANCE PLAN for the upcoming year
- Define developmental activities in section 6. UPCOMING YEAR DEVELOPMENT PLAN for the upcoming year.
- Forward completed draft to your Manager who will schedule time to meet for one-on-one discussion.
Step 2: Managers complete the following after receiving information from Team Member:
Optional: Solicit feedback via 360 Degree Feedback on Team Member’s performance from other leaders, peers, and internal customers, etc. (where applicable). A manager should evaluate team member performance based on the individuals’ job responsibilities, key performance results, job factors, and the feedback received.
- Complete ratings for 1. CORE VALUES and add MANAGER COMMENTS
- Complete MANAGER RATING on 2. YEAR END PERFORMANCE REVIEW and answer questions 1 & 2 in the 3. YEAR END PERFORMANCE DISCUSSION
- Complete section 4. YEAR END INDIVIDUAL DEVELOPMENT ACTIVITIES
- Review Objectives in section 5. UPCOMING YEAR PERFORMANCE PLAN for the upcoming year with Team members.
- Review developmental activities in section 6. UPCOMING YEAR DEVELOPMENT PLAN for the upcoming year with Team members.
- Meet for one-on-one discussion with Team member to review and discuss, add/change any comments, finalize Upcoming Year Objectives and/or current year’s Overall Ratings and sign form.
Following completion of the Performance & Development Plan, return this form to Human Resources. Completed forms must be signed by the Team Member and the Manager.
*Please note: Before a salary change can take place, a Performance Review and Individual Development Plan must have been completed within the last twelve months.
