I am fully aware that he/she will be engaging in physical exercise and that the use of exercise equipment, the studio facilities, training and instruction could cause
injury.
I am voluntarily allowing him/her to participate in these activities and
assume responsibility for any risk and/or injury that may result.
I agree to waive any claims or rights I might otherwise have to sue the studio,
its owners, employees, or other related agents.
I also understand that he/she will conduct himself/herself in a responsible manner and treat the facility, its members and staff in an appropriate manner. All minors ages 13-16 must always supervised by an adult while using Bars & Belles Studio.
Failure to comply with the rules and regulations of the facility or the terms of this
agreement will result in the immediate termination of his/her participation in
the sessions.
I further acknowledge that this parental waiver will remain on file with Bars &
Belles for a period of six (6) months at which time a new parental waiver will
need to be applied.
