    Thank you for choosing Highland Veterinary Clinic!  The following Boarding Admission form is offered as an efficient and user-friendly check-in process for our pet guests and their families.  Once submitted by email, your check-in sheet will be attached to your pet's account and printed for your pet's identification This Express Check-In is for the convenience of Highland Veterinary Clinic, Evansville, IN clients and patients who have established an account at Highland Veterinary Clinic, either through a previous boarding experience, or as a an established veterinary medical client.  Completion of the form is not adequate toward making a reservation for boarding.  If you have yet to make a reservation for your pet, please call 812-867-6652.  
  • We will attach a copy of these responses to your pet's record and can provide a printed copy at your request. Admission time on reservation day is 7:30am-5:30pm on the day your pet is scheduled to arrive. Please see our boarding guide to better prepare your pet for boarding.

  • Medications and supplements should be brought into the hospital in their original packaging. Insulin is sensitive to both light and temperature, and should be provided in the bottle and the box, and a new insulin syringe should be provided by the pet owner for every dose which we are to administer. It is not necessary, however to provide a sharps container. If you would like for us to use a pill minder we can do place the tablets or capsules into the pill minder for you, but should have the original package with the label and DVM instruction on the bottle.

  • General Terms:  Highland Veterinary Clinic will exercise responsible care for the safety of your pet, and to keep the boarding premises safe and properly enclosed. Pets will be fed and watered regularly, and housed in clean, safe quarters. Highland Veterinary Clinic, it's doctors and staff, cannot guarantee against accidents, and we cannot be liable for loss or damage caused by or to our pet guests at this facility. Owner agrees to be solely responsible for any and all attacks or damage caused by owners' pet while it is at this resort.

  • Payment/Nonpayment: Highland Veterinary Clinic charges for boarding space by the night . Boarding is a luxury service, offered to our clients at a highly competitive rate. Payment is required upon release of the animal(s) regardless of who is picking up the patient(s), and patient(s) will not be released to anyone if all boarding services are not paid in full at time of pick up. A valid payment method may be left on file attached to your account in the form of Visa/Mastercard, AMEX, or Discover.  We accept PayPal and CareCredit payments online. Further boarding fees will be accrued for each night that the patient remains in our care.

  • Check in/out Times: Check out time is by 6:00PM each evening. Any pet checked out before 6:00PM will not be charged the boarding fee for the night of check out. Any pet checked out after 6:00PM will be charged for the night of check out. Check in and check out times are only during normal business hours. There are no Sunday check outs.  If a pet is not claimed within 5 days after the above discharge date and we have not been contacted by his/her owner about a delay, or extension to the discharge date, we must assume that the pet is abandoned and are authorized by law to re-home the pet as we deem best and necessary. Animal abandonment laws and collection procedures, including additional collection/attorney fees will be enforced.  Highland Veterinary Clinic is not responsible for any personal items left with the patient.  Highland Veterinary Clinic provides blankets and may provide chewable toys (such as Kong pup-scicles, rawhide, or handmade cat-toys to patients) I acknowledge that proper hospital-style housekeeping does not allow for blankets, bedding, leashes, and non-consumable toys etc. and if I leave such items at the hosptial, Highland veterinary Clinic is not responsible for lost or damaged personal items:

  • Medications & Safety: Medications, supplements, or other items will be administered as directed if a technician or nurse is required to administer said medications outside of rounds.  This includes Insulin and injectable medications. but medications must be presented in their original containers with instructions for administration.  If behavioral medication such as trazodone, gabapentin, or acepromazine is necessary due to my pet's emotional health, or for humane treatment or handling of my pet, I give my permission to Highland Veterinary Clinic to administer such medications. Your signature authorizes Highland Veterinary Clinic in its discretion, give first aid, medication, or other attention we deem it necessary for the health, and safety of your pet. Highland Veterinary Clinic is authorized by the owner to provide veterinary care, including emergency care, at the owner's expense. If we believe that your pet is in need of care, time permitting we will attempt to contact you before providing that care, but this document serves as our authorization to provide veterinary care for your pet in the event we are unable to reach the owner. The owner is responsible for expenses of veterinary care, whether or not you have been reached in advance. Your signature on this authorization permits Highland Veterinary Clinic to make reasonable care decisions regarding your pet; and the owner agrees to pay for all costs incurred for such treatment. In the unlikely event that a pet passes away while a guest of Highland Veterinary Clinic, we will contact you and discuss your options of body care with you I also authorize highland Veterinary Clinic to transport my pet away from the facility in the event of a natural or man-made disaster, or an emergency involving the building and grounds, if evacuation is deemed necessary

  • Monitoring & Staff Safety: Staffing at Highland Veterinary Clinic varies by season and days. If at any time a staff member is not present on the premises, the hospital is locked, monitored by an off-site alarm company, For staff safety, dogs are not walked between the hours of 9 pm and 6 am.  Certain staff members wear remote alarm systems when caring for your pet and may not be approached outside of business hours.  If at any time a staff member feels threatened or uncomfortable, they are fully authorized to utilize their panic wear which will provide a police response:

  • I further understand that In the event of nonpayment, fraudulent payment, bounced checks, or any violation of any prior agreed payment arrangement, the entire balance shall be considered in default with the addition of any and all banking, collection fees, collection agency and/or attorney fees, necessary to the amount due to Highland Veterinary Clinic without any relief whatever from Valuation and Appraisement Laws. I further understand that my check may be presented electronically at any time for validation or deposit.

    I further certify that my pet is in fair to good health and has not been ill with any communicable condition nor to my knowledge been exposed to any communicable diseases within the last 30 days. Moreover, I certify to the accuracy of all information given about my pet and have discussed any previous signs of aggression or threatening behavior toward any person or animal. I have read and understand the entire boarding contract.
