Swimming Liability Waiver
ADULT WAIVER FOR MINOR PARTICIPANT

RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT FOR SWIM WITH NOA

In consideration for allowing my child(ren) to participate in the related events and activities with NOA ATLAS, I hereby warrant and agree to the following including but not limited to:

1. I have checked that having a private swim instructor conduct lessons in the home pool being used, is covered with the pool owner's homeowner insurance policy
2. I am the parent/guardian that will be present during all lessons to assist in ensuring a safe environment for my child(ren)
3. I am familiar with and accept, on behalf of myself and my minor child(ren) that there is the risk of serious injury and death in participation in swimming
4. I understand, that at no time will Noa Atlas be liable for any loss or damage to the property, or injury to the child(ren) including death or any type of injury which takes place during the lesson
5. I have satisfied myself and believe that my minor child(ren) is physically, emotionally and mentally able to participate in this event
6. I understand, and will instruct my minor child(ten), that all applicable rules for participation must be followed and that at all times the sole responsibility for personal safety remains with the minor child(ren) and the parent
7. I authorize Noa Atlas to consent to emergency medical treatment in accordance with the best interests of my minor child(ren), should I not be present at the relevant time to grant consent myself
8. I understand and agree that the agreements listed above apply for all swimming lessons

I HAVE READ AND UNDERSTAND THIS AGREEMENT AND I AM AWARE THAT BY SIGNING THIS AGREEMENT I AM WAIVING CERTAIN SUBSTANTIAL LEGAL RIGHTS WHICH MY MINOR CHILD/REN AND I MAY HAVE AGAINST THE RELEASEES.

Cancellation/Makeup Policy
- 24 hour notice of class cancellation is required for a refund/credit
- All makeups need to be used during the current seasonal session.
- Class will be at the time slot you have booked. I cannot guarantee that extra time will be granted.
- Missed classes or last minunte cancellations will result in a fee of 50% of the class price.
Booking / Payment
- Booking are only finalized after payment for 4 lessons is sent to swimwithnoa@gmail.com
- You will be contacted with your finalized time and date for swim lessons as we are still receiving requests
Additional Swim Policies
- The pool must be kept to a minimum of 82F to safely conduct outdoor swim lessons
- Lessons can contiue when drizzling but must be canceled in hard rain & strom conditions
- Rain day cancellations will be called 30 minutes prior to the lesson
- Photos and videos ARE permitted - feel free to forward any photos to (647) 828 4662 to be shared on the SwimwithNoa instagram
- Parent & tot group classes are capped at 5 swimmers
- Refrain from having additional recreational pool toys
- Pool is scimmed and free of leaves, bugs etc.
