Business Card, Prescription Pad and Name Tag Request
Use this form to request Business Cards, Prescription Pads, and Name Tags.

Marketing will process these requests within the following timelines:
- Business Cards: Actioned within 48 hours
- Prescription Pads: Ordered within 48 hours
- Name Tags: May be delayed to align with bulk order quantities (typically 10+ requests) to manage costs.
Reminder: Satori Health will cover the cost of up to two (2) name tags per qualifying employee. Additional name tags can be purchased by employees at their own expense. Marketing recommends using a temporary name tag provided by People & Culture until a bulk order is placed. The cost of a purchased name tag varies depending on the total number ordered at the time and ranges between $8 and $25 per name tag.

Important: If you have not received your business cards within seven (7) days of the order being placed, please contact Marketing so we can follow up with the supplier. While we can confirm an order has been printed and shipped, we cannot guarantee delivery timelines. If there are any errors with your business cards or prescription pads, please inform Marketing immediately so we can address the issue.
