Media Permission & Release Form
Youth For Christ Puerto Rico (YFCPR) shares photos and videos to celebrate what God is doing through our programs and to inform families and supporters about our mission.

This release applies to all YFCPR programs and events (such as Revive, Revive on Ramey, Campus Life, Young Adults House of Prayer, Mission Teams, Camps, and other YFCPR-related activities).
I grant permission to Youth For Christ Puerto Rico, Inc. (YFCPR) to photograph, video record, and/or audio record me and/or my child(ren) during YFCPR events.

I authorize YFCPR to use, reproduce, publish, and distribute these materials for ministry and promotional purposes, including but not limited to:
- Social media
- Website content
- Printed promotional materials
- Newsletters and presentations
- Ministry and fundraising communications
I Understand and Agree:
1. Use Authorization – YFCPR may use these images and recordings for the purposes listed above.
2. No Compensation – Neither I nor my child will receive payment for their use.
3. Editing & Respectful Use – Materials may be edited for clarity or design but will not be used in a misleading or inappropriate manner and will reflect YFCPR’s mission and values.
4. Privacy Protection – No sensitive personal information (such as home address, phone number, or full name) will be published without additional written consent.
5. Release of Liability – I release YFCPR, its employees, volunteers, and authorized representatives from liability related to the use of these materials.
6. Discontinuation of Use – I may request in writing that YFCPR discontinue future use of a photo or video; however, this does not apply to materials already printed or distributed.
