2024 NTPGA JUNIOR TOUR REFUND REQUEST FORM

Prep & Medalist Tour
All cancellations should be submitted by canceling online. This form should only be used when requesting a refund after the cancellation period has passed and is only applicable for NTPGA Prep and Medalist tournaments.

In case of medical or family emergencies, written requests for refunds must be submitted to the Northern Texas PGA office NO MORE THAN THREE (3) DAYS AFTER THE COMPLETION OF THE JUNIOR TOUR EVENT. Each request will be considered separately on its own merit and will be reviewed by the NTPGA Junior Tour Staff. If the request has been approved, a refund will be applied to the player’s account credit balance with the NTPGA Junior Tour.
