LETTER OF INTENT TO APPLY
United Way of the Ohio Valley will be accepting Letter of Intent forms to apply for a one year UWOV Impact Grant. Included with the Intent Form is detailed information of the impact areas targeted for funding and grant application requirements. Please proceed with completing the Letter of Intent Form only if your program meets the outlined requirements.

Upon review and if invited to submit a full application, your program will then be sent an application to complete.

Do you have case managers that will assist clients in reaching their goals that lead to reducing poverty? If so, explain case management activities.

What is your estimated timeline for the implementation of your impact project/services?

What is your estimate of funds needed to complete the proposal?

Please submit a rough draft budget of how you will spend the funds you will be requesting.

Please include if you will be collaborating with any other organizations/programs for your impact project. Also, please include if you are requesting funds from any other potential funders for your impact project.
