    TO APPLICANTS:

    Thank you for your interest in traveling with One Hope Missions.  Please take the time to complete the application form and releases in its entirety to ensure your consideration as a ministry team member. 

    One Hope Missions asks for a fairly in-depth amount of information.  Some information required may be rather personal to you, however, reasonable considering the scope of such a ministry trip. 

    Because our team is comprised of individuals from a variety of backgrounds and experiences, it is imperative that One Hope Missions has prior knowledge of each applicant’s personal information before approval can be granted for team participation.  Please be assured that all details provided are kept strictly confidential within the leadership of One Hope Missions.


    Please complete the mission application, supplemental form and releases in its          entireity. 

    If you are under 18 years of age - a parental consent form will be emailed to you.  This form must be completed and returned to One Hope Missions via email one month prior to travel date.

    A mandatory deposit of $350 (USD) reserves your space on this ministry team            pending review and approval of your application.  The Deposit must be sent when the application is submitted in order for your application to be reviewed. 

    Space is limited and acceptance is based on a first come first served basis.  If the deposit is not received with your application, then your application will not be processed.  

    Please be sure to submit the Deposit and Payment Information Form along with this application.

    A color copy of your passport is a requirement, if you do not yet have a passport you may submit your application now and then send us a copy once you have received it. 

    Please note, it takes 6-8 weeks for standard passport processing, and 2-4 weeks for RUSH passport processing.  

    Visit www.travel.state.gov/passport for more information about passport application.  Passport is required no later than 90 days before date of travel.  A copy of your passport must be sent via email to onehopemissionsinc@gmail.com as soon as it is obtained.

    APPLICATION PROCESS

    Thank you for prayerfully considering joining one of our mission teams.  The Mission Director and trip coordinators will review all applications.  Consideration will be based on the information you provide with priority consideration given to completed applications received on or before the due date.  Submission of an application does not guarantee a position on a team.  Applicants will be notified once a determination has been made. 

    Please note that each team member must be at least 14 years of age or older and a born again Christian who is comfortable sharing their faith.  Each team member must have a valid U.S passport and submit a completed application.  Each team member is responsible for paying for his/her own costs by personal contributions, donations from family and friends and fund-raising.  Please review the Support/Fund/Donations explanation which is included in this online form.

    Missionary travel by its own nature offers an unfamiliar and unique environment.  Thus risks of injury, bodily harm, or death, in addition to property damage or loss are inherent.  I understand that by my participating in the ministry of One Hope Missions, I am indicating my acceptance of these possible risks. 

    I/parent(s),guardian do hereby release One Hope Missions, its Board of Directors, staff members and volunteers from any liability whatsoever arising out of any injury, bodily harm, death or personal property damage, or loss which may be sustained during the course of involvement with this missions trip.

    1. I/parent(s)/guardian are responsible for providing medical and accident insurance while participating in this mission trip.
    2. I/parent(s)/guardian understand that if I/my child/ward are operating a motor vehicle abroad, I/parent(s)/guardian will be responsible to obtain insurance for travel within the country of travel.

    DISCIPLINE POLICY AND MEDIA POLICY

    I VOLUNTARILY SUBMIT MYSELF TO THE FOLLOWING DISCIPLINARY PROTOCOL

    “If your brother sins, go and show him his fault in private, if he listens to you, you have won your brother.  But if he does not listen to you, take one or two more with you so that BY THE MOUTH OF TWO OR THREE WITNESSES EVERY FACT MAY BE CONFIRMED.  If he refuses to listen to them, tell it to the church and if he refuses to listen even to the church, let him be to you as a Gentile and a tax collector” – Matthew 18:1517

    It is the intent of One Hope Missions to follow the Biblical patterns of discipline within the confines of all international ministry trips.  The goal of One Hope Missions is to create a safe, positive, supportive and healthy environment in order to minister to the people of the country visited.  We recognize that ministry team members must be in correct relationship with God and with others, in order to ensure completion of mission objectives.  The consequences of one’s sins or disobedience have the potential to bring confusion, and destruction to any ministry trip.  We desire to come along side each ministry team member in loving correction only when necessary.  All compliance with any disciplinary action by One Hope Missions  is non-negotiable.  Below are procedures that will be followed by One Hope Missions leadership,  If any disciplinary action is necessary.  To avoid any misunderstanding please read the outlined procedures below, and sign the consent form directly below.  By consenting to the following you agree to receive correction, public rebuke and or removal if deemed absolutely necessary by One Hope Missions leadership.  If issues or sin or disobedience come to light the steps below will be followed to bring resolution to the situation.

    1.        If you have a problem with an individual you are to lovingly approach that person without going to any other ministry team member (in other words do not gossip or try to get people on your side).  Attempt to bring understanding and resolution to the conflict.  If it is with someone of the opposite sex, please talk with him or her in a place where others are present, but cannot hear your conversation.  Many times what you may consider a problem is simply a misunderstanding and bringing it to their attention often brings resolution. 

    2.        If you find no resolution after you have conversed with the individual, the individuals involved are required to discuss the problem with a ministry team leader.  The ministry team leader should be able to determine what the problem is, who is at fault and bring closure to the situation.

    3.        If the ministry team leaders discovers that there has been no closure to the situation, there will be another meeting with the parties involved, the ministry team leader and the trip coordinator in order to bring closure to the difficult situation. 

    4.        If the ministry team leader and event coordinator find any individual to be in rebellion to correction a senior One Hope Missions representative will be informed.  A senior One Hope Missions representative will bring definite closure to the situation, in which all parties will be present to hear the final conclusion of the matter.  In extreme situations possible conclusions may include an individual returning home within 24 hours (at their own expense) or being brought before the whole ministry team for public correction.  If absolutely necessary, the ministry team will be informed not to have any personal contact with the individual throughout the remainder of the trip.  The individual will not be permitted to eat, sleep or travel with any ministry team member. 

    MEDIA RELEASE

      I,  agree (IN CONSIDERATION OF MY BEING ACCEPTED BY ONE HOPE MISSIONS FOR PARTICIPATION AS A MINISTRY TEAM MEMBER.                                                                                   

    In consideration of my being accepted by One Hope Missions for participation as a ministry team member for the following trip - I agree to ADHERE TO THE FOLLOWING MEDIA POLICY. 

     One Hope Missions often take photographs and video footage on ministry trips which are then used as needed for One Hope Missions  advertising, promotional material, web page and publications.  In signing below you fully authorize One Hope Missions to use video or photographs taken of you in any or all of the above mentioned materials.  In addition to this any photo or video footage personally acquired on the trip will be at the disposal of One Hope Missions should they request it.

    If you choose to do fundraising to pay for your trip please follow the below guidelines.

    DONATIONS FROM FRIENDS AND FAMILY FOR THE APPLICANT/TEAM MEMBER - must be sent to the applicant directly.

    CANCELLATION/REFUND POLICY

    One Hope Missions reserves the right to cancel, reschedule and/or change the details of the Mission Trip and in-country itinerary without notice.  Although rare, this is most likely to occur due to the following conditions. 

    Travel conditions or conditions in the host country are deemed unsafe, due to civil unrest, natural disasters, man-made disasters, wide-spread illness, pandemic or other extraordinary events.

    Once you submit your application your deposit will be non-refundable.  One Hope Missions reserves the right to deny any application.  If you are not accepted for the team, your deposit will not be refunded.  If for any reason a team member cancels or the trip is cancelled by One Hope Missions , any amount paid towards the trip will be forfeited. 

    All payments must be received by set due dates, if payments are not received on time , your trip is subject to cancellation without refund.  If your trip is cancelled for any reason and your airfair has already been purchased by One Hope Missions you may be able to utliize the airfare tickets for another One Hope Missions trip within a year.
