I understand that I’ll be contacted for authorization if the cost of replacement parts exceeds estimate & if I don’t authorize parts replacement, my regulator will be returned to me unassembled.
I understand that parts and labor are separate charges and fees are subject to change without notice.
I understand that an additional $15.00 per stage Excessive Corrosion fee will apply if excessive salt or verdigris deposits are present that require pre-cleaning or additional cleaning to disassemble.
I authorize replacement of Hoses and Mouthpieces if needed during repair and/or service.
House of Scuba guarantees all parts installed by the Repair facility will perform satisfactorily under conditions of normal use for a period of 90 days after date of repair completion. Serviced items and goods left for more than 60 days after completion of contracted work become the property of House of Scuba, INC. House of Scuba, INC is not responsible for items left for a time period exceeding the 60 days. Payment for service must be rendered at time of pick-up. Serviced item becomes the property of House of Scuba, INC if payment is not received within 60 days of repair completion. Used parts will be presented to the customer at time of payment; the customer can obtain these parts if so desired. However, these parts are not considered serviceable and are not be used under any circumstances. House of Scuba, INC nor the Repair Department nor the Repair Technicians can be held responsible if these parts are used by customer any other person, in any manner and for any reason. Parts and Labor prices are subject to change at any time at the discretion of House of Scuba and without notice.
